摘  要:目的 研究血铅水平与儿童血红蛋白(HB)、红细胞平均血红蛋白浓度(MCHC)及皮质体感诱发电位(SEP)的关系。方法 选取2021年4月至2023年4月于广州市第十二人民医院体检及因疾病就诊的儿童1 020例为研究对象,其中血铅值>100μg/L范围选取伴有厌食、睡眠障碍、多动及注意力下降等临床症状儿童104例为症状组;不伴上述症状且血铅值50~100μg/L儿童102例为非症状组,另选取血铅值<50μg/L的儿童101例为对照组。对比三组血铅水平在各年龄段(分为1~3岁,4~6岁,7~10岁)的分布情况;对比三组FEP、Hb、MCHC值变化;对比三组SEP测定结果;对比三组事件相关电位(ERP)检测;采用Pearson分析血铅与HB、MCHC、SEP(包括P45、N35、N60波)的相关性。结果 1~3岁儿童血铅水平比较:对照组>非症状组>症状组,4~6岁儿童血铅水平比较:非症状组>症状组>对照组,7~10岁儿童血铅水平比较:症状组>对照组>非症状组,差异均有统计学意义(P<0.05)。FEP值比较:症状组>非症状组>对照组,Hb、MCHC值比较:对照组>非症状组>症状组,差异均有统计学意义(P<0.05)。三组P15、P25、N20三波值比较差异无统计学意义(P>0.05),而P45、N35、N60波值比较:症状组>非症状组>对照组,差异均有统计学意义(P<0.05)。N200波幅、P300波幅值比较:对照组>非症状组>症状组,N200潜伏期、P300潜伏期值比较:症状组>非症状组>对照组,差异均有统计学意义(P<0.05)。经Pearson直线相关性分析:血铅水平与HB、MCHC呈负相关,与P45、N35、N60呈正相关(P<0.05)。结论 HB、MCHC、SEP值与儿童体内血铅水平具有密切关系,可通过检测HB、MCHC与SEP值为临床诊疗儿童体内铅中毒提供可靠资料。Objective To study the relationship between blood lead level and childrens hemoglo-bin(HB),mean corpuscular hemoglobin concentration(MCHC)and cortical somatosensory evoked potential.Methods 1020 cases of children who received medical examinations and received treatment due to diseases in Guangzhou 12th Peoples Hospital from April 2021 to April 2023 were selected as the research object,among which,104 cases of children with lead of>100μg/L,who had clinical symptoms such as anorexia,sleep distur-bance,hyperactivity and decreased attention were selected as the symptom group;102 children without the above symptoms and whose blood lead level was 50~100μg/L were included in the non-symptomatic group,and another 101 cases of children with a blood lead level<50μg/L were selected as the control group.The distri-bution of blood lead levels of different ages(divided into 1~3 years,4~6 years,7~10 years old)in the three groups was compared;the changes of FEP,Hb and MCHC values in the three groups were compared;the SEP measurement results in the three groups were compared.The event-related potential(ERP)detection among the three groups were compared.The correlation between blood lead and HB,MCHC,SEP(including P45,N35,N60 waves)was analyzed by Pearson.Results Comparison of blood lead levels in children aged 1~3:control group>non~symptomatic group>symptomatic group,comparison of blood lead levels in children aged 4~6:non~symptomatic group>symptomatic group>control group,comparison of blood lead levels in children aged 7~10:symptomatic group>control group>non~symptomatic group,and the difference was statistically signifi-cant(P<0.05).Comparison of FEP values:symptomatic group>non~symptomatic group>control group,and comparison of Hb and MCHC values:control group>non~symptomatic group>symptomatic group,and the difference was statistically significant(P<0.05).There was no statistically significant difference in the three wave values of P15,P25 and N20 among the three groups(P>0.05),while the comparison of the wave values of P45,N
