复律宁颗粒治疗气阴两虚型房颤的临床观察

摘　　要：目的观察复律宁颗粒治疗气阴两虚型房颤的疗效及安全性,评估中医加载治疗的优势和特点。方法纳入80例辨证为气阴两虚证的心房颤动患者,以1∶1分配原则随机分入复律宁颗粒+西药观察组(观察组)以及安慰剂模拟+西药观察组(对照组),每日2次,每次1包,两组疗程均为4周,评价内容包括治疗前后中医症候、心悸发作频率、累计发作时间、24 h心电图(Holter)、血清N末端B型利钠肽原(NT-proBNP)、安全性指标。结果观察组中医症候总疗效为84.9%,显著优于对照组(57.1%,P﹤0.05),其中在心悸、自汗、盗汗、口干、疲乏程度改善上观察组优于对照组(P﹤0.05)。观察组患者临床症状改善总有效率78.8%,显著优于对照组(51.4%,P﹤0.05),其中观察组患者心悸发作频率、持续时间显著减少(P﹤0.05)。两组在治疗前后对于NT-proBNP的改变均不明显,且组间无明显差异(P>0.05)。两组治疗过程中均未出现明显不良反应。结论复律宁颗粒治疗气阴两虚型房颤安全有效,其能有效改善患者中医症候,且能降低房颤发作频率、持续时间、降低心室率。Objective To observe the efficacy and safety of Fulvning granules on deficiency of Qi and Yin with Atrial Fibrillation-and to evaluate the advantages and characteristics of TCM combined therapy.Methods Patients in qi-yin Deficiency group were divided into Fulvning Decoction+Western medicine treatment group and placebo granule+Western medicine treatment group ac-cording to random principle,40 cases in each group,One pack twice a day.Treatment of Chinese medicine symptoms,palpita-tions frequency,cumulative attack time,24 hours electrocardiogram(Holter),serum N-terminal pro-B type natriuretic pep-tide(NT-proBNP),safety indicators were evaluated before and after treatment.Results The total curative effect of TCM syn-drome in the observation group was 84.9%,which was significantly better than that in the control group(57.1%,P﹤0.05).Compared with the control group,the treatment group improved the symptoms of palpitations,spontaneous sweat,night-swea-ting,dry mouth and fatigue(P<0.05).The total response rate of patients in the observation group was 78.8%,which was sig-nificantly better than that in the control group(51.4%,P<0.05),and the frequency and duration of palpitations were signifi-cantly decreased(P<0.05).Neither group changed the NT-proBNP significantly before and after the treatment(P>0.05).No significant adverse effects occurred during treatment in both groups.Conclusion Fulvning granules is safe and effective in the treatment of atrial fibrillation,and it can effectively improve the TCM syndrome and reduce the frequency,duration and ventricu-lar rate of atrial fibrillation。
