摘　　要：目的:探究不同测量方法分析前交叉韧带(anterior cruciate ligament,ACL)磁共振成像(magnetic resonance imaging,MRI)信号表现的可靠性,并分析年龄和性别对结果的影响。方法:回顾性分析100例于本院行膝关节MRI检查的患者,所有纳入患者均无前交叉韧带损伤史。收集所有患者的人口统计学资料与膝关节MRI图片,利用ImageJ软件分析ACL与后交叉韧带(posterior cruciate ligament,PCL)、背景、股四头肌肌腱、髌腱和腓肠肌内侧头肌腱的灰度值,并分别以PCL、背景、股四头肌肌腱、髌腱和腓肠肌内侧头肌腱的灰度值作为参考,计算ACL信号强度比(signal intensity ratio,SIR),分析测量计算方法的可靠性,以及测量结果的相关因素。结果:本研究共纳入100例患者,5种测量计算方法观察者自我重复测量一致性均达到较好的水平(r>0.75);观察者间一致性均达到中等至较好的水平(r>0.7)。5种测量方式的结果,仅以PCL为参考的测量结果与年龄和性别均无关,另外4种测量计算方法均受性别影响,且女性SIR低于男性SIR(P<0.05)。结论:5种前交叉韧带磁共振信号表现测量方式均有中等至较好的一致性和可重复性表现,但仅以PCL为参考的测量计算方式得到的SIR结果均不受年龄和性别的影响。Objective To explore the reliability of different measurement methods in analyzing the magnetic resonance imaging(MRI)signal intensity of anterior cruciate ligament(ACL),and to analyze the effect of age and gender.Methods A total of 100 patients receiving knee MRI in our hospital were analyzed retrospectively,and all of them had no history of anterior cruciate ligament injury.Their de⁃mographic data and knee MRI pictures were collected to analyze the gray values of ACL to the posteri⁃or cruciate ligament(PCL),background,quadriceps tendon,patellar tendon,and the medial head of the gastrocnemius using the ImageJ software,respectively.Then,the measured gray value was used as a reference respectively to calculate the ACL signal intensity ratio(SIR).The reliability of the 5 mea⁃surements and their related factors were analyzed.Results For all patients,the test-retest reliability reached a good level(r>0.75)for all five measurement&calculation methods,with their inter-observ⁃er reliability above the average(r>0.7).For the five measurement methods,all results,except that of us⁃ing PCL as a reference,were related to age and gender,with the signal intensity ratio of the female lower than the male(P<0.05).Conclusion All the five measurement methods are of moderate or above consistency and repeatability,but all SIR results are affected by age and sex,except that using PCL as a reference.
