髋关节腔封闭注射形成“超声关节造影”对前上盂唇撕裂的诊断价值

摘　　要：目的:探讨髋关节腔封闭注射形成“超声关节造影”对于髋关节前上盂唇撕裂的诊断价值。方法:回顾性分析64例髋关节腔诊断性封闭注射治疗患者的超声、磁共振成像(magnetic resonance imaging,MRI)及关节镜手术资料。以关节镜手术和MRI诊断结果作为复合金标准。评估封闭注射形成“超声关节造影”对髋关节前上盂唇撕裂的诊断效能。应用McNemar’s检验比较封闭前、后超声诊断髋关节前上盂唇撕裂的敏感度、特异度及准确度。结果:髋关节封闭注射前、后超声诊断盂唇撕裂的敏感度由79.17%提升至85.42%,准确度由73.43%提升至76.56%,但差异均无统计学意义(P值分别为0.250和0.687)。封闭注射后,40.63%病例盂唇内裂隙显示更加明显。结论:髋关节封闭注射后,结合注射液形成的“关节腔造影”效果再次进行超声检查,前上盂唇撕裂诊断的敏感度及准确度有增高的趋势。Objective To explore the diagnostic value of“ultrasound arthrography”resulted from pain⁃killing injections for anterosuperior acetabular labral tears(ALT).Methods The ultrasound,magnetic res⁃onance imaging(MRI)and arthroscopy data of 64 patients treated with painkilling injections were ana⁃lyzed retrospectively.The arthroscopy and MRI results served as compound gold standard to evaluate the diagnostic performance of“ultrasound arthrography”resulted from painkilling injections for anterosu⁃perior ALT.The sensitivity,specificity and accuracy before and after painkilling injections was com⁃pared using McNemar’s test.Results The sensitivity and accuracy of ultrasonic diagnosis for ALT be⁃fore and after painkilling injections improved from 79.17%and 73.43%,to 85.42%and 76.56%,respec⁃tively.However,the difference was not significant(P=0.250,0.687).Moreover,after painkilling injec⁃tions,more obvious labral cleft was observed in 40.63% of the ALT cases.Conclusion Combined with the“arthrography effect”resulted from painkilling injections,the sensitivity and accuracy of ultrasonic diagnosis for anterosuperior ALT after hip joint painkilling injections tend to increase.
