乳腺磁共振成像肿块形态特征在良恶性鉴别诊断中的应用价值

摘　　要：目的:研究乳腺磁共振成像(MRI)肿块形态特征在良恶性鉴别诊断中的应用价值。方法:收集河南中医药大学第一附属医院2020年8月—2021年8月451例进行乳腺MRI检查患者的临床资料,回顾性分析经病理证实的91例乳腺肿块病变患者的MRI资料,其中良性肿块病变40例,恶性肿块病变51例。对比乳腺良恶性肿块的最大径、形状和边缘等形态特征,并分析肿块形态特征的诊断效能。结果:良恶性组肿块最大径分别为(15.15±6.21)mm、(22.76±11.57)mm,差异有统计学意义(t=3.758,P<0.001)。良性组肿块形状以卵圆形为主(62.5%),恶性组肿块形状以为不规则形为主(56.9%),差异有统计学意义(χ^(2)=30.018,P<0.001)。良性组肿块边缘以清晰光整为主(80.0%),恶性组肿块边缘以为不规则(51.0%)和毛刺(45.1%)为主,差异有统计学意义(χ^(2)=58.525,P<0.001)。肿块最大径、形状、边缘对乳腺良恶性肿块诊断的敏感度分别为77%、57%、96%,特异度分别为63%、85%、80%,准确率分别为70%、69%、89%。结论:乳腺MRI肿块形态特征在肿块良恶性鉴别诊断中有一定应用价值,肿块边缘敏感度、特异度、准确率均较高。Objective:To study the application value of morphological characteristics of breast mass on magnetic resonance imaging in benign and malignant differentiation.Methods:Clinical data were collected from 451 patients who underwent breast MRI in the hospital from August 2020 to August 2021.The MRI data of 91 patients with pathologically confirmed breast mass lesions were retrospectively analyzed,including 40 benign mass lesions and 51 malignant mass lesions.Morphological features such as maximum diameter,shape and margins of benign and malignant breast masses were compared,and the diagnostic efficacy of morphological features of the masses was analyzed.Results:The maximum diameter of the masses in the benign and malignant groups were(15.15±6.21)mm and(22.76±11.57)mm,respectively,with statistically significant differences(t=3.758,P<0.001).The shape of the masses in the benign group was predominantly ovoid(62.5%)and the shape of the masses in the malignant group was predominantly irregular(56.9%),with statistically significant differences(χ^(2)=30.018,P<0.001).The margin of the benign group was predominantly clear and shiny(80.0%),while the margin of the malignant group was predominantly irregular(51.0%)and burred(45.1%),with a statistically significant difference(χ^(2)=58.525,P<0.001).The sensitivity of the maximum diameter,shape,and margin of the mass for the diagnosis of benign and malignant breast masses was 77%,57%,and 96%,the specificity was 63%,85%,and 80%,and the accuracy was 70%,69%,and 89%.Conclusion:The morphological features of breast MRI masses have some application in the differential diagnosis of benign and malignant masses,and the sensitivity,specificity and accuracy of mass margins are high.
