摘  要:目的分析反复输血患者白三烯(LT)、涎液化糖链抗原-6(KL-6)、可溶性白细胞介素-2受体(SIL-2R)表达水平变化与输血不良反应的相关性。方法选取2019年7月至2021年7月我院收治的反复输血患者70例为研究组,另外选取同期70例在我院体检的健康志愿者为对照组,采用酶联免疫法(ELISA)检测白三烯B4(LTB4)、白三烯C4(LTC4)、KL-6、SIL-2R表达,分析LTB4、LTC4、KL-6、SIL-2R在反复输血患者的表达变化、四者相关性及对不良反应的预测价值。结果与对照组相比,研究组LTB4、LTC4、KL-6、SIL-2R水平升高,差异有统计学意义(P<0.05)。与未出现不良反应组相比,不良反应组LTB4、LTC4、KL-6、SIL-2R水平升高,差异有统计学意义(P<0.05)。LTB4、LTC4、KL-6、SIL-2R之间Pearson相关性分析显示,LTB4、LTC4呈正相关(r=0.769,P=0.001);LTB4、KL-6呈正相关(r=0.421,P=0.001);LTB4、SIL-2R呈正相关(r=0.566,P=0.001);LTC4、KL-6呈正相关(r=0.437,P=0.001);LTC4、SIL-2R呈正相关(r=0.560,P=0.001);KL-6、SIL-2R呈正相关(r=0.488,P=0.001)。ROC曲线显示,与LTB4、LTC4、KL-6、SIL-2R单项诊断相比,四项联合诊断对输血不良反应的预测价值较高。结论LTB4、LTC4、KL-6、SIL-2R在反复输血患者体内表达升高,且随着不良反应的发生,表达逐渐升高,且LTB4、LTC4、KL-6、SIL-2R之间具有相关性,临床上可根据其表达预测不良反应的发生。Objective To analyze leukotriene(LT),krebs von den lungen-6(KL-6),and soluble interleukin-2 receptor(SIL-2R)correlation between changes in expression levels and adverse blood transfusion reactions.Methods From July 2019 to July 2021,70 patients with repeated blood transfusions admitted to our hospital were selected as research group(RG),and 70 healthy volunteers who received physical examination in our hospital were selected as control group(CG).Enzyme-linked immunoassay(ELISA)was used to detect white blood leukotriene B4(LTB4),leukotriene C4(LTC4),KL-6,SIL-2R expression,analysis of LTB4,LTC4,KL-6,SIL-2R expression changes in patients with repeated blood transfusion,four relevance and predictive value of adverse reactions.Results Compared with CG,LTB4,LTC4,KL-6,and SIL-2R in RG increased with statistically significant difference(P<0.05).Compared with the non-adverse reaction group,LTB4,LTC4,KL-6,and SIL-2R in the adverse reaction group increased,and difference was statistically significant(P<0.05).Pearson correlation analysis among LTB4,LTC4,KL-6 and SIL-2R showed positive correlation between LTB4 and LTC4(r=0.769,P=0.001).LTB4 and KL-6 were positively correlated(r=0.421,P=0.001).LTB4 and SIL-2R were positively correlated(r=0.566,P=0.001).LTC4 and KL-6 were positively correlated(r=0.437,P=0.001).LTC4 and SIL-2R were positively correlated(r=0.560,P=0.001).KL-6 and SIL-2R were positively correlated(r=0.488,P=0.001).The ROC curve showed that compared with the single diagnosis of LTB4,LTC4,KL-6,and SIL-2R,combination of four items had a higher predictive value for adverse blood transfusion reactions(P=0.001).Conclusion Expressions of LTB4,LTC4,KL-6,and SIL-2R were increased in patients with repeated transfusion,and gradually increased with the occurrence of adverse reactions,and there was a correlation between LTB4,LTC4,KL-6,and SIL-2R,which could be used to predict the occurrence of adverse reactions clinically.
