摘 要:目的探讨促甲状腺激素(TSH)正常的2型糖尿病患者的TSH与骨代谢的相关性。方法回顾性选取2016年1月至2021年11月合肥市第一人民医院滨湖院区内分泌科甲状腺功能正常的239例2型糖尿病患者作为研究对象,根据TSH三分位数分为三组,比较各组临床指标的差异,并比较TSH与各指标的相关性。结果三组患者病程、年龄、低密度脂蛋白(LDL)、总Ⅰ型胶原氨基端延长肽(PINP)、总胆固醇(TC)、β胶原特殊序列(β-CTX)、游离甲状腺素(FT_(4))、胰岛素抵抗指数比较,差异有统计学意义(P<0.05)。Spearman相关性分析显示,TSH与PINP呈正相关,TSH与钙、β-CTX、OC无相关性。结论2型糖尿病正常范围偏高的TSH可以提高骨形成指标,骨形成活跃,有望减少骨质疏松的发生。Objective To investigate the correlation between thyroid stimulating hormone(TSH)and bone metabolism in type 2 diabetes patients with normal TSH.Methods A total of 239 patients with type 2 diabetes with normal thyroid function in the Department of Endocrinology of Hefei First People's Hospital Lakeside District from January 2016 to November 2021 were retrospectively selected as the study objects,and they were divided into three groups according to the TSH tripartite.The differences of clinical indicators among each group were compared,and the correlation between TSH and each indicator was compared.Results There were significant differences in disease course,age,low density lipoprotein(LDL),procollagen typeⅠN-terminal propeptide(PINP),total cholesterol(TC),beta-isomerized C-telopeptide(β-CTX),free thyroid hormones(FT_(4))and insulin resistance index among three groups(P<0.05).Spearman correlation analysis showed that TSH was positively correlated with PINP,but TSH was not correlated with calcium,β-CTX,and OC.Conclusion High TSH in the normal range of type 2 diabetes can increase bone formation index and active bone formation,which is expected to reduce the occurrence of osteoporosis.
