摘 要:目的探讨miR-145在自身免疫甲状腺炎(AIT)患者外周血中表达情况,并分析其与Th1/Th2比值的关系。方法回顾性选取2020年1月至2021年12月武汉市中医医院收治的245例AIT患者病例资料,作为研究对象(AIT组);选取同时期本院体检健康者250名为对照组。通过实时荧光定量PCR法检测所有受试者血浆中miR-145的表达情况,以流式细胞术对外周血中Th1细胞比例、Th2细胞比例进行检测,并计算Th1/Th2比值;采用酶联免疫吸附分析检测所有受试者血浆中细胞因子γ干扰素(IFN-γ)、白细胞介素-4(IL-4)水平。Pearson相关性分析miR-145与Th1、Th2、Th1/Th2比值及IL-4水平的相关性。结果AIT组患者血浆miR-145表达水平为0.33±0.11,低于对照组(1.02±0.36),差异有统计学意义(P<0.05)。AIT组患者外周血Th1细胞比例、Th1/Th2比值分别为(7.78±1.88)%、2.69±0.84,均高于对照组[(3.64±0.96)%、0.88±0.26],Th2细胞比例为(3.17±1.89)%,低于对照组[(5.48±2.04)%],差异均有统计学意义(P<0.05)。AIT组患者血浆中IFN-γ水平为(35.73±10.82)pg/mL,高于对照组[(22.47±6.15)pg/mL],IL-4水平为(3.67±1.06)ng/mL,低于对照组[(6.74±2.04)ng/mL],差异均有统计学意义(P<0.05)。Pearson分析显示,miR-145表达水平与Th1细胞比例、Th1/Th2比值、IFN-γ、IL-4负相关,与Th2细胞比例、IL-4正相关(P<0.05)。结论AIT患者血浆中miR-145低表达,Th1/Th2比值升高,miR-145可能经对Th1/Th2比值产生影响参与AIT发生。Objective To investigate the expression of miR-145 in peripheral blood of patients with autoimmune thyroiditis(AIT),and the relationship between miR-145 and Th1/Th2 ratio was analyzed.Methods A total of 245 patients with AIT in Wuhan Traditional Chinese Medicine Hospital from January 2020 to December 2021 were retrospectively selected as the study objects(AIT group),and 250 health people in the same hospital at the same time were selected as the control group.The expression of miR-145 in the plasma of all subjects was detected by real-time quantitative PCR,the proportion of Th1 cells and Th2 cells in peripheral blood were measured by flow cytometry and calculate the ratio of Th1/Th2;the levels of interferon-γ(IFN-γ)and interleukin-4(IL-4)in plasma of all subjects were detected by enzyme-linked immunosorbent assay,and the correlation between miR-145 and Th1,Th2,Th1/Th2 and IL-4 levels was analyzed by Pearson correlation analysis.Results The expression level of plasma miR-145 in the AIT group was 0.33±0.11,which was lower than that in the control group(1.02±0.36),and the difference was statistically significant(P<0.05).The Th1 cell ratio and Th1/Th2 ratio of peripheral blood in AIT group were(7.78±1.88)%and 2.69±0.84,respectively,which were higher than those in the control group[(3.64±0.96)%and 0.88±0.26],the Th2 cell ratio was(3.17±1.89)%,which was lower than that in the control group[(5.48±2.04)%],and the differences were statistically significant(P<0.05).The level of plasma IFN-γin the AIT group was(35.73±10.82)pg/mL,which was higher than that in the control group[(22.47±6.15)pg/mL],while the level of IL-4 was(3.67±1.06)ng/mL,which was lower than that in the control group[(6.74±2.04)ng/mL],the differences were statistically significant(P<0.05).Pearson analysis showed,the expression level of miR-145 was negatively correlated with Th1 cell proportion,Th1/Th2 cell ratio,IFN-γand IL-4,and positively correlated with Th2 cell proportion and IL-4(P<0.05).Conclusion The expression of miR-145 in plasma of
