金属棒熨烫与球形电极治疗宫颈环形电切术出血临床疗效比较

摘　　要：目的比较宫颈环形电切术(LEEP)出血治疗中球形电极治疗与金属棒熨烫的临床疗效。方法回顾性选取2020年2月至2023年2月西电集团医院妇产科LEEP出血患者140例,依据治疗方法分为金属棒熨烫组、球形电极治疗组两组,各70例。比较两组患者的临床疗效、出血量、出血持续时间、宫颈修复时间、阴道排液时间、创面愈合时间和并发症发生情况。结果金属棒熨烫组患者的总有效率为94.29%,明显高于球形电极治疗组(68.57%),差异有统计学意义(P<0.05)。金属棒熨烫组患者的出血量<月经量比率为62.86%,明显高于球形电极治疗组(22.86%),金属棒熨烫组患者出血量≈月经量、>月经量比率分别为34.29%、2.86%,均明显低于球形电极治疗组(57.14%、20.00%),差异均有统计学意义(P<0.05)。金属棒熨烫组患者的出血持续时间≤7 d比率为54.29%,明显高于球形电极治疗组(22.86%),8~14 d、15~35 d、≥36 d的比率分别为31.43%、14.29%、0,均明显低于球形电极治疗组(34.29%、40.00%、2.86%),差异均有统计学意义(P<0.05)。金属棒熨烫组患者的宫颈修复时间半个月比率为28.57%,明显高于球形电极治疗组(8.57%),1个月比率为71.43%,明显低于球形电极治疗组(91.43%),差异有统计学意义(P<0.05)。金属棒熨烫组患者的阴道排液时间、创面愈合时间均明显短于球形电极治疗组,差异均有统计学意义(P<0.05)。金属棒熨烫组患者的并发症发生率为2.86%,明显低于球形电极治疗组(22.86%),差异有统计学意义(P<0.05)。结论金属棒熨烫治疗LEEP出血的临床疗效明显优于球形电极治疗,可减少患者出血量,缩短出血持续时间、宫颈修复时间、阴道排液时间及创面愈合时间,降低并发症发生率。Objective To compare the clinical curative effect of spherical electrode treatment and metal rod ironing in the treatment of hemorrhage during loop electrosurgical excision procedure of cervix(LEEP).Methods A total of 140 cases of bleeding patients undergoing LEEP in Department of Obstetrics and Gynecology,XD Group Hospital from February 2020 to February 2023 were retrospectively selected and divided into two groups according to treatment methods:metal rod ironing group and spherical electrode treatment group,70 cases in each group.Clinical efficacy,blood loss,bleeding duration,cervical repair time,vaginal drainage time,wound healing time and complications of the two groups of patients were compared.Results The total effective rate of patients in the metal rod ironing group was 94.29%,which was significantly higher than that in the spherical electrode treatment group(68.57%),the difference was statistically significant(P<0.05).The ratio of bleeding volume to menstrual volume in the metal rod ironing group was 62.86%,which was significantly higher than that in the spherical electrode treatment group(22.86%),the bleeding volume≈menstrual volume and>menstrual volume ratio in the metal rod ironing group were 34.29%,2.86%,which were significantly lower than those in the spherical electrode treatment group(57.14%,20.00%),the differences were statistically significant(P<0.05).The rate of bleeding duration≤7 days in the metal rod ironing group was 54.29%,which was significantly higher than that in the spherical electrode treatment group(22.86%),and the rates of 8-14 days,15-35 days,≥36 days were 31.43%,14.29%,0,which were significantly lower than those in the spherical electrode treatment group(34.29%,40.00%,2.86%),the differences were statistically significant(P<0.05).The half month rate of cervical repair time in the metal rod ironing group was 28.57%,which was significantly higher than that in the spherical electrode treatment group(8.57%),and the one month rate was 71.43%,which was significantly lower than that
