胰岛素注射妊娠期糖尿病患者出院准备度现状及影响因素分析

摘　　要：目的 调查胰岛素注射妊娠期糖尿病患者出院准备度(RHDS)现状及所接受出院指导质量(QDTS),探讨影响胰岛素注射妊娠期糖尿病患者RHDS的因素。方法 选取2022年1月-2023年2月在南京市某三级甲等医院住院后行胰岛素治疗的妊娠期糖尿病患者,采用一般资料调查表、出院准备度量表(RHDS)、出院指导质量量表(QDTS),出院当天对胰岛素注射妊娠期糖尿病患者进行调查。并采用单因素及多元线性回归分析影响胰岛素注射妊娠期糖尿病患者RHDS的因素。结果 胰岛素注射的妊娠期糖尿病患者RHDS得分为193.73±12.84分;QDTS得分为163.29±7.99分。出院准备度与出院指导质量呈线性相关(P<0.05)。经过单因素筛选,多元线性回归分析显示,文化程度、受孕方式、不良嗜好、每日活动时长、出院指导质量是妊娠期糖尿病患者RHDS的独立影响因素(P<0.05)。结论 胰岛素注射的妊娠期糖尿病患者RHDS及QDTS处于较高水平;文化程度较低、非自然受孕、有不良嗜好、自主活动时间短的胰岛素注射妊娠期糖尿病患者出院准备度较差。Objective To investigate the current status of readiness for discharge scale(RHDS)and the quality of discharge instructions received(QDTS)in insulin-injected gestational diabetes mellitus patients,and to explore the factors aff ecting RHDS in insulin-injected gestational diabetes mellitus patients.Methods Patients with gestational diabetes mellitus who were treated with insulin after hospitalisation in a Class III Grade A hospital in Nanjing from January 2022 to February 2023 were selected,and the general information questionnaire,readiness for discharge scale(RHDS),and quality of discharge instructions scale(QDTS)were used to investigate the patients with gestational diabetes mellitus on the day of their discharge from the hospital.Factors affecting RHDS in insulin injection gestational diabetic patients were also analysed using univariate and multivariate analysis.Results The RHDS score of insulin-injected gestational diabetic patients was 193.73±12.84 points;the QDTS score was 163.29±7.99 points.Discharge readiness was linearly correlated with quality of discharge instructions(P<0.05).After univariate screening,multifactorial linear regression analysis showed that literacy,mode of conception,adverse hobbies,daily activity hours,and quality of discharge instructions were independent influences on RHDS in patients with gestational diabetes mellitus(P<0.05).Conclusion RHDS and QDTS of gestational diabetes mellitus insulin injection patients with insulin injection were at a higher level;discharge readiness of gestational diabetes mellitus patients with lower literacy,unnatural conception,bad hobbies,and short duration of autonomous activities was poor.
