摘  要:目的分析冰冻血小板活化、功能以及代谢情况变化。方法随机选择30名单采血小板献血者的新鲜单采血小板30袋留样10 mL为新鲜血小板组,冰冻血小板30袋42℃水浴融化留样10 mL为冰冻血小板组(解冻后)。比较两组的PAC-1、CD62P、血栓弹力图(TEG)参数、血小板计数(PLT)、平均血小板体积(MPV)、大型血小板比率(P-LCR)、血小板分布宽度(PDW)、血小板pH及生化指标。结果冰冻血小板组的PAC-1、CD62P明显高于新鲜血小板组(P<0.05)。两组的凝血反应时间(R)比较,差异无统计学意义(P>0.05);冰冻血小板组的血块形成时间(K)明显长于新鲜血小板组,ANG、血块最大强度(MA)明显低于新鲜血小板组(P<0.05)。冰冻血小板组的PLT低于新鲜血小板组,MPV、P-LCR、PDW高于新鲜血小板组(P<0.05)。两组的pH比较,差异无统计学意义(P>0.05);冰冻血小板组的葡萄糖水平低于新鲜血小板组,乳酸、乳酸脱氢酶水平高于新鲜血小板组(P<0.05)。结论冰冻血小板在冰冻保存、解冻复融过程出现血小板损伤,膜脂质、血小板形态和生化性质发生不可逆性改变,导致血小板正常功能受损,从而会影响其临床输注效果。Objective To analyze the changes of frozen platelet activation,function and metabolism.Methods A total of 30 apheresis platelet donors were randomly selected,30 bags of fresh apheresis platelets were retained for 10 mL as fresh platelet group,and 30 bags of frozen platelets were dissolved in 42℃water bath for 10 mL as frozen platelet group(after thawing).The PAC-1,CD62P,thrombus elastograph(TEG)parameters,platelet count(PLT),mean platelet volume(MPV),platelet-large cell ratio(P-LCR),platelet distribution width(PDW),platelet pH and biochemical indicators were compared between the two groups.Results The PAC-1 and CD62P in the frozen platelet group were significantly higher than those in the fresh platelet group(P<0.05).There was no significant difference in coagulation reaction time(R)between the two groups(P>0.05);the kinetics time(K)of the frozen platelet group was significantly longer than that of the fresh platelet group,and the ANG and maximum amplitude(MA)were significantly lower than those of the fresh platelet group(P<0.05).The PLT of the frozen platelet group was lower than that of the fresh platelet group,and the MPV,P-LCR and PDW were higher than those of the fresh platelet group(P<0.05).There was no significant difference in pH between the two groups(P>0.05);the glucose level in the frozen platelet group was lower than that in the fresh platelet group,and the levels of lactic acid and lactate dehydrogenase were higher than those in the fresh platelet group(P<0.05).Conclusion Platelet damage occurs during frozen preservation,thawing and rethawing of frozen platelets,and irreversible changes occur in membrane lipids,platelet morphology and biochemical properties,resulting in impaired normal platelet function,which can affect its clinical transfusion outcome.
