摘 要:目的探讨乙型肝炎病毒(HBV)RNA和HBV DNA在慢性乙型肝炎(CHB)及乙型肝炎病毒相关肝细胞癌(HBV-HCC)中的表达。方法以2021年6月至2023年6月在西安交通大学第一附属医院消化内科或感染科就诊的82例CHB患者和61例HBV-HCC患者为研究对象。检测两组的HBV DNA、HBV RNA、丙氨酸氨基转移酶(ALT)、门冬氨酸氨基转移酶(AST)、碱性磷酸酶(ALP)及γ-谷氨酰转移酶(GGT)及乙型肝炎表面抗原(HBsAg)。比较两组的实验室指标,分析两组的HBV DNA与HBV RNA检出情况及相关性,分析HBV DNA与HBV RNA对HBV-HCC的诊断效能。结果CHB组的HBV DNA、HBV RNA载量及HbsAg阳性率高于HBV-HCC组(P<0.05);CHB组的ALT、AST、ALP及GGT水平低于HBV-HCC组(P<0.05)。两组的HBV RNA阳性率均略高于HBV DNA;对阳性数据做散点图结果显示,CHB组的HBV RNA载量高于HBV DNA(P<0.05);对HBV DNA和HBV RNA阳性数据行相关性分析结果显示,两组患者的HBV DNA与HBV RNA均呈正相关(P<0.05)。受试者工作特征(ROC)曲线结果显示,HBV RNA诊断HBV-HCC的曲线下面积(AUC)高于HBV DNA。结论HBV RNA在CHB进展为HBV-HCC的过程中具有比HBV DNA更优的临床监测价值。Objective To investigate the expression of hepatitis B virus(HBV)RNA and HBV DNA in chronic hepatitis B(CHB)and hepatitis B virus human hepatocellular carcinoma(HBV-HCC).Methods From June 2021 to June 2023,82 patients with CHB and 61 patients with HBV-HCC who were admitted in the gastroenterology department or infectious diseases department of the First Affiliated Hospital of Xi'an Jiaotong University were selected as the study subjects.The HBV DNA,HBV RNA,alanine aminotransferase(ALT),aspartate aminotransferase(AST),alkaline phosphatase(ALP),γ-glutamyltransferase(GGT)and hepatitis B surface antigen(HBsAg)were detected in the two groups.The laboratory indexes of the two groups were compared,the detection and correlation of HBV DNA and HBV RNA in the two groups were analyzed,and the diagnostic efficacy of HBV DNA and HBV RNA for HBV-HCC was analyzed.Results The HBV DNA,HBV RNA load and HbsAg positive rate in the CHB group were higher than those in the HBV-HCC group(P<0.05);the levels of ALT,AST,ALP and GGT in the CHB group were lower than those in the HBV-HCC group(P<0.05).The positive rate of HBV RNA in the two groups was slightly higher than that of HBV DNA;the results of scatter plot of positive data showed that the HBV RNA load in the CHB group was higher than that of HBV DNA(P<0.05);correlation analysis of HBV DNA and HBV RNA positive data showed that HBV DNA and HBV RNA were positively correlated in the two groups(P<0.05).The results of the receiver operating characteristic(ROC)curve showed that the areas under curve(AUC)of HBV RNA in the diagnosis of HBV-HCC was higher than that of HBV DNA.Conclusion HBV RNA has better clinical monitoring value than HBV DNA in the progression of CHB to HBV-HCC.
