摘  要:目的探究复方异丙托溴铵联合压力调节容量控制通气(PRVC)治疗慢性阻塞性肺疾病急性加重期(AECOPD)的效果。方法选择2017年3月至2022年3月收治的90例AECOPD患者作为研究对象,按照随机数字表法将其分为对照组(n=45)和观察组(n=45)。对照组接受常规对症治疗及PRVC,观察组在对照组治疗方案上加施复方异丙托溴铵。比较两组的治疗效果。结果治疗后,观察组的气道阻力(Raw)、气道峰压(PIP)、气道平台压(Pplat)、气道平均压(mPaw)低于对照组(P<0.05)。治疗后,观察组的平静呼吸时膈肌移动度(DM_(QB))、深吸气位膈肌移动度(DM_(DB))、膈肌厚度(TD)高于对照组(P<0.05)。治疗后,观察组的临床肺部感染评分(CPIS)、Borg评分低于对照组(P<0.05)。结论复方异丙托溴铵联合PRVC治疗AECOPD可改善呼吸力学参数水平,促进膈肌功能恢复,减少肺部感染,缓解呼吸困难。Objective To explore the effect of compound ipratropium bromide combined with pressure regulated volume control(PRVC)in the treatment of acute exacerbation of chronic obstructive pulmonary disease(AECOPD).Methods A total of 90 patients with AECOPD admitted from March 2017 to March 2022 were selected as the research objects and divided into control group(n=45)and observation group(n=45)according to the random number table method.The control group received conventional symptomatic treatment and PRVC,and the observation group was treated with compound ipratropium bromide on the basis of the treatment plan of the control group.The therapeutic effects of the two groups were compared.Results After treatment,the airway resistance(Raw),peak airway pressure(PIP),airway plateau pressure(Pplat)and mean airway pressure(mPaw)in the observation group were lower than those in the control group(P<0.05).After treatment,the diaphragm mobility during quiet breathing(DM_(QB)),deep inspiratory diaphragm mobility(DM_(DB))and diaphragm thickness(TD)in the observation group were higher than those in the control group(P<0.05).After treatment,the Clinical Pulmonary Infection Score(CPIS)and Borg score of the observation group were lower than those of the control group(P<0.05).Conclusion Compound ipratropium bromide combined with PRVC in the treatment of AECOPD can improve the levels of respiratory mechanics parameters,promote the recovery of diaphragm function,reduce pulmonary infection and relieve dyspnea.
