柴芍六君汤治疗慢性乙型肝炎肝纤维化的效果及对丙氨酸氨基转移酶、总胆红素水平的影响

摘　　要：目的分析柴芍六君汤治疗慢性乙型肝炎肝纤维化的临床效果。方法选取2020年1月至2022年1月我院收治的80例慢性乙型肝炎肝纤维化患者,根据随机数字表法将其分为对照组(n=40,恩替卡韦)和研究组(n=40,恩替卡韦+柴芍六君汤)。比较两组的治疗效果。结果研究组的治疗总有效率显著高于对照组(P<0.05)。治疗后,研究组的门静脉内径、脾脏厚度小于对照组(P<0.05)。治疗后,研究组的miR-122水平高于对照组,miR-181a、血管内皮细胞生长因子(VEGF)、内皮素-1(ET-1)水平及辅助性T细胞17(Th17)/调节性T细胞(Treg)低于对照组(P<0.05)。治疗后,研究组的透明质酸(HA)、层黏蛋白(LN)、Ⅲ型前胶原(PCⅢ)、Ⅳ型胶原(Ⅳ-C)、丙氨酸氨基转移酶(ALT)、总胆红素(TBil)水平低于对照组(P<0.05)。结论柴芍六君汤应用于慢性乙型肝炎肝纤维化效果显著,不仅能够改善患者的肝纤维化程度与肝功能,还能调节血清指标,有助于促进疾病转归。Objective To analyze the clinical effect of Chaishao Liujun decoction in the treatment of chronic hepatitis B liver fibrosis.Methods A total of 80 patients with chronic hepatitis B liver fibrosis admitted in our hospital from January 2020 to January 2022 were selected and divided into control group(n=40,entecavir)and study group(n=40,entecavir+Chaishao Liujun decoction)according to the random number table method.The therapeutic effects of the two groups were compared.Results The total effective rate of treatment in the study group was significantly higher than that in the control group(P<0.05).After treatment,the portal vein diameter and spleen thickness of the study group were smaller than those of the control group(P<0.05).After treatment,the miR-122 level in the study group was higher than that in the control group,and the miR-181a,vascular endothelial growth factor(VEGF),endothelin-1(ET-1)levels and T helper cell 17(Th17)/regulatory T cells(Treg)were lower than those in the control group(P<0.05).After treatment,the levels of hyaluronic acid(HA),laminin(LN),precollagen typeⅢ(PCⅢ),typeⅣcollagen(Ⅳ-C),alanine aminotransferase(ALT)and total bilirubin(TBil)in the study group were lower than those in the control group(P<0.05).Conclusion Chaishao Liujun decoction applied in chronic hepatitis B liver fibrosis has a significant effect.It can not only improve the liver fibrosis degree and liver function of patients,but also regulate serum indicators,which is helpful to promote the prognosis of the disease.
