摘  要:目的分析单孔加一孔腹腔镜手术(SILS+1)治疗子宫肌瘤患者的临床效果。方法回顾性分析2018年1月至2021年12月诊治的子宫肌瘤患者66例,其中根据不同的治疗方式分为30例单孔腹腔镜子宫肌瘤剔除术组(SILS组),36例单孔加一孔腹腔镜子宫肌瘤剔除术组(SILS+1组)。两组手术时间、术中出血量、手术成功率、术后并发症发生率及术后24 h的疼痛视觉模拟评分(VAS)等指标作比较分析。结果和SILS组比较,SILS+1组的手术时间更短,术中出血量更少,手术成功率更高(均P<0.05),在手术并发症和术后24 h的VAS评分方面,两组对比无差异(均P>0.05)。结论单孔加一腹腔镜子宫肌瘤挖除术能有效缩短手术时间,减少出血量,不会增加手术并发症,安全可行。Objective To investigate the efficacy of single-incision plus one-port laparoscopic surgery for uterine myomectomy.Methods Retrospective analysis of 66 patients with uterine fibroids diagnosed and treated from January 2018 to December 2021,in which they were divided into 30 cases of single-port laparoscopic myomectomy group(SILS group)and 36 cases of single-port plus one laparoscopic myomectomy group(SILS+1 group)according to different treatment modalities.The surgical time,intraoperative bleeding,surgical success rate,incidence of postoperative complications and visual analogue score(VAS)of pain at 24 h postoperatively were compared and analysed between the two groups.Results Compared with the SILS group,the SILS+1 group had a shorter operative time,less intraoperative bleeding,and a higher surgical success rate(all P<0.05).In terms of surgical complications and VAS scores at 24 h postoperatively,there was no difference in the comparison between the two groups(both P>0.05).Conclusions SILS+1 myomectomy can effectively shorten the operation time,reduce intraoperative blood loss and improve the success of the operation rate,no increased surgical complications,is safe and feasible.
