颈胸段带状疱疹性神经痛患者血清IL-2、NPY及P物质水平与病情程度的相关性

摘　　要：目的:分析颈胸段带状疱疹性神经痛(PHN)患者血清白细胞介素2(IL-2)、神经肽(NPY)及P物质(SP)水平与病情程度的相关性。方法:选择105例颈胸段带状疱疹PHN患者、50例带状疱疹(HZ)未遗留神经痛的患者及50名健康体检者为研究对象,分别纳入PHN组、单纯HZ组和对照组,PHN组根据症状严重程度进一步分为轻度组、中度组和重度组。比较PHN组、单纯HZ组、对照组及PHN不同亚组血清IL-2、NPY、SP水平,分析血清IL-2、NPY、SP水平与疼痛程度及IL-2、NPY、SP之间的相关性。结果:各组血清IL-2、NPY、SP水平比较:PHN组>单纯HZ组>对照组(P<0.05),PHN不同病情程度比较:重度组>中度组>轻度组(P<0.05);Pearson相关分析显示,PHN患者血清IL-2、NPY、SP水平与疼痛视觉模拟评分法(VAS)评分均正相关(P<0.05),血清IL-2与NPY、SP及NPY与SP均正相关(P<0.05);多元线性回归分析显示,IL-2、NPY、SP均为PHN患者疼痛程度的独立影响因素(P<0.05)。结论:PHN患者IL-2、NPY、SP与疼痛程度正相关,且IL-2、NPY、SP均对颈胸段PHN患者疼痛程度具有独立影响。Objective:To analyze the correlation between the serum levels of Interleukin-2(IL-2),Neuropeptide Y(NPY),and Substance P(SP)with the severity of disease in patients with cervicothoracic herpes zoster postherpetic neuralgia(PHN).Methods:A total of 105 patients with PHN,50 patients with herpes zoster(HZ)without residual neuralgia,and 50 healthy subjects were included in the PHN group,the HZ group alone,and the control group,respectively.Based on symptom severity,the PHN group was further divided into mild,moderate,and severe subgroups.The serum levels of IL-2,NPY,and SP across these groups and subgroups were compared.The correlation between the serum levels of IL-2,NPY,SP,and pain severity as well as between IL-2,NPY,and SP were analyzed.Results:Both the PHN and simple HZ groups showed higher serum levels of IL-2,NPY,and SP compared to the control group(P<0.05),and the PHN group was higher than the simple HZ group(P<0.05).In the PHN subgroups,the serum levels of IL-2,NPY,and SP were higher in the moderate and severe groups than those in the mild group(P<0.05),and the severe group was higher than the moderate group(P<0.05).Pearson correlation analysis revealed a positive correlation between serum levels of IL-2,NPY,SP,and Visual Analogue Scale(VAS)pain scores in PHN patients(P<0.05).There was also a positive correlation between serum levels of IL-2 with NPY,SP,and between NPY and SP(P<0.05).Multivariate linear regression analysis indicated that IL-2,NPY,and SP independently influence the pain severity in PHN patients(P<0.05).Conclusion:Serum levels of IL-2,NPY,and SP in PHN patients positively correlate with pain severity,and each of IL-2,NPY,and SP has an independent impact on the pain severity of cervicothoracic PHN patients.
