摘 要:目的:探讨单髁人工膝关节置换术(UKA)术中髌骨周围去神经化治疗累及髌股关节膝关节骨性关节炎的近期疗效。方法:选取112例累及髌股关节膝关节骨性关节炎患者为研究对象,根据治疗方式不同分为观察组和对照组,每组各56例。观察组患者采取UKA术中髌骨周围去神经化治疗;对照组患者采取常规UKA术治疗。比较两组患者治疗前、治疗3个月后的疼痛情况[视觉模拟(VAS)评分]、膝关节炎症情况[美国骨关节炎指数(WOMAC)评分]、髌股关节功能(髌股关节Kujala评分)及并发症发生情况。结果:治疗3个月后,观察组患者VAS、WOMAC评分低于对照组(P<0.05),Kujala评分高于对照组(P<0.05)。两组患者并发症发生情况比较,差异无统计学意义(P>0.05)。结论:UKA术中髌骨周围去神经化治疗累及髌股关节的膝关节骨性关节炎患者可有效改善患者膝关节疼痛症状,促进髌股关节功能恢复。Objective:To investigate the short-term effect of peripatellar deneralization in the treatment of knee osteoarthritis involving patellofemoral joint during unicondylar artificial knee arthroplasty(UKA).Methods:112 patients with knee osteoarthritis involving patellofemoral joint were selected as the study subjects,and divied into the observation group and the control group according to the different treatment,with 56 cases in each group.Patients in the observation group were treated with peripatellar denervation during UKA,while the patients in the control group were treated with UKA.The changes in pain status[Visual Analogue Score(VAS)],knee inflammation[Western Ontario and McMaster Universities Osteoarthritis Index(WOMAC)]and patellofemoral function[patellofemoral joint score(Kujala score)]before treatment and after 3 months of treatment and occurrence of complications during treatment were compared between the two groups of patients.Results:After 3 months of treatment,the scores of VAS and WOMAC in observation group were lower than those in control group,while the Kujala score was higher than that in control group(P<0.05).There were no significant differences in the complications during treatment between both groups of patients(P>0.05).Conclusion:Peripatellar denervation during UKA has a significant efficacy on patients with knee osteoarthritis involving patellofemoral joint,and it can improve the recovery of patellofemoral joint function.
