摘　　要：目的探讨经肾上盏入路经皮肾镜碎石术(PCNL)治疗复杂型肾下盏肾结石的效果及安全性。方法选取2020年5月至2023年4月我院收治的90例接受PCNL治疗的复杂型肾下盏结石患者,按照穿刺入路的不同分为观察组(经肾上盏入路)与对照组(经肾下盏入路)。比较两组的治疗效果以及并发症。结果观察组的结石清除率为86.67%,高于对照组的60.00%(P<0.05)。观察组的手术时间短于对照组,Hb下降水平低于对照组(P<0.05)。观察组的并发症发生率为6.67%,低于对照组的31.11%(P<0.05)。结论经肾上盏入路PCNL治疗复杂型肾下盏结石可提高结石清除率,缩短手术时间,治疗安全性较高。Objective To explore the effect and safety of percutaneous nephrolithotomy(PCNL)through transrenal superior calyx approach for the treatment of complex lower renal caliceal stones.Methods A total of 90 patients with complex lower renal caliceal stones who underwent PCNL treatment in our hospital from May 2020 to April 2023 were selected and divided into the observation group(transrenal superior calyx approach)and the control group(transrenal inferior calyx approach)according to different puncture approaches.The therapeutic effects and complications of the two groups were compared.Results The stone clearance rate in the observation group was 86.67%,higher than 60.00%in the control group(P<0.05).The operation time in the observation group was shorter than that in the control group,and the decreased level of Hb was lower than that in the control group(P<0.05).The incidence of complications in the observation group was 6.67%,lower than 31.11%in the control group(P<0.05).Conclusions PCNL through transrenal superior calyx approach in the treatment of complex lower renal caliceal stones can improve the stone clearance rate,shorten the operation time,and has high treatment safety.
