单采血小板和冷沉淀凝血因子配合应用对难治性产科大出血患者凝血功能的影响

摘　　要：目的探讨单采血小板与冷沉淀凝血因子配合输注治疗难治性产科大出血患者的效果。方法70例难治性产科大出血患者随机分为两组,对照组应用冷沉淀凝血因子输注治疗,观察组应用单采血小板配合冷沉淀凝血因子输注治疗,比较两组的凝血功能、止血效果及免疫功能。结果治疗后,观察组的APTT、PT、TT均低于对照组,FIB高于对照组,CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)水平均高于对照组(P<0.05)。观察组的平均止血时间短于对照组,24 h有效止血率高于对照组(P<0.05)。结论单采血小板和冷沉淀凝血因子配合应用可改善难治性产科大出血患者的凝血功能,提高止血效果,且对患者免疫功能的损伤程度更低。Objective To explore the effect of combined infusion of apheresis platelets and cryoprecipitate coagulation factors in the treatment of patients with refractory obstetric massive hemorrhage.Methods 70 patients with refractory obstetric massive hemorrhage were randomly divided into two groups.The control group was treated with cryoprecipitate coagulation factors infusion,and the observation group was treated with combined infusion of apheresis platelets and cryoprecipitate coagulation factors.The coagulation function,hemostatic effect and immune function were compared between the two groups.Results After treatment,the APTT,PT and TT of the observation group were lower than those of the control group,the FIB was higher than that of the control group,and the levels of CD3t,CD4^(+)and CD4^(+)/CD8^(+)were higher than those of the control group(P<0.05).The average hemostasis time of the observation group was shorter than that of the control group,and the effective hemostasis rate of 24 h was higher than that of the control group(P<0.05).Conclusions The combined application of apheresis platelets and cryoprecipitate coagulation factors can improve the coagulation function of patients with refractory obstetric massive hemorrhage,improve the hemostatic effect,and has less damage to the immune function of patients.
