胫骨平台骨折患者实施单侧与双侧锁定钢板内固定治疗的临床效果

摘　　要：目的:分析单侧、双侧锁定钢板内固定治疗胫骨平台骨折的临床效果。方法:选取2018年1月—2022年1月泰州市中医院骨科收治的胫骨平台骨折患者60例作为观察对象,随机分为对照组与观察组,各30例。对照组给予单侧锁定钢板内固定治疗,观察组给予双侧锁定钢板内固定治疗,比较两组治疗效果。结果:两组手术时间、术中出血量比较,差异无统计学意义(P>0.05);观察组术后负重时间、骨折愈合时间、住院时间短于对照组,差异有统计学意义(P<0.001)。术后1、3、6个月,观察组膝关节功能评分高于对照组,差异有统计学意义(P<0.001)。术后,两组患者生活质量评分高于术前,观察组高于对照组,差异有统计学意义(P<0.05)。观察组并发症总发生率低于对照组,差异有统计学意义(P=0.038)。结论:在对胫骨平台骨折患者实施治疗中,给予双侧锁定钢板内固定所获疗效更佳,该方式有助于缩短术后恢复时间,促进膝关节功能恢复,提高生活质量,安全性较高。Objective:To analyze the clinical effect of unilateral and bilateral locking plate internal fixation in treatment of tibial plateau fracture.Methods:A total of 60 patients with tibial plateau fracture admitted to the Department of Orthopedics of Taizhou Hospital of Traditional Chinese Medicine from January 2018 to January 2022 were selected as observation objects,and they were randomly divided into control group and observation group,with 30 cases in each group.The control group was treated with unilateral locking plate internal fixation,and the observation group was treated with bilateral locking plate internal fixation.The treatment effects of the two groups were compared.Results:There was no significant difference in operation time and intraoperative blood loss between the two groups(P>0.05).The postoperative weight-bearing time,fracture healing time and hospital stay time in the observation group were shorter than those in the control group,and the differences were statistically significant(P<0.001).At 1,3 and 6 months after surgery,the knee function score in the observation group was higher than that in the control group,and the difference was statistically significant(P<0.001).After surgery,the quality of life scores in the two groups were higher than those before surgery,and the observation group was higher than the control group,and the difference was statistically significant(P<0.05).The total incidence of complications in the observation group was lower than that in the control group,and the difference was statistically significant(P=0.038).Conclusion:In the treatment of patients with tibial plateau fracture,bilateral locking plate internal fixation obtains better efficacy,which is helpful to shorten the postoperative recovery time,promote the recovery of knee joint function,improve the quality of life,and have high safety.
