缝合止血与双极电凝止血在腹腔镜卵巢囊肿剥除术患者中的应用效果比较

摘　　要：目的:比较缝合止血与双极电凝止血在腹腔镜卵巢囊肿剥除术患者中的应用效果。方法:选取2021年1月至2022年12月该院收治的104例卵巢囊肿患者进行前瞻性研究,按照随机数字表法将其分为对照组和观察组各52例。两组均行腹腔镜卵巢囊肿剥除术治疗,对照组采用双极电凝止血,观察组采用缝合止血,两组术后均随访1个月。比较两组手术相关指标(手术时间、术中出血量、首次下床活动时间及住院时间)水平,手术前后卵巢功能指标[卵巢动脉收缩期峰值血流速度(PSV)、基础窦卵泡数(AFC)]水平、性激素指标[卵泡刺激素(FSH)、黄体生成素(LH)、雌二醇(E_(2))]水平,以及并发症发生率。结果:观察组手术时间长于对照组,术中出血量多于对照组,差异有统计学意义(P<0.05);术后1个月,两组PSV、AFC和E2水平均低于术前,但观察组高于对照组,两组FSH、LH水平均高于术前,但观察组低于对照组,差异有统计学意义(P<0.05);两组首次下床活动时间、住院时间及并发症发生率比较,差异均无统计学意义(P>0.05)。结论:缝合止血应用于腹腔镜卵巢囊肿剥除术患者可提高卵巢功能指标水平,改善性激素指标水平,效果优于双极电凝止血,但手术时间较长,术中出血量较多。Objective:To compare application effects of suture hemostasis and bipolar electrocoagulation hemostasis in patients undergoing laparoscopic ovarian cystectomy.Methods:A prospective study was conducted on 104 patients with ovarian cysts admitted to this hospital from January 2021 to December 2022.They were divided into control group and observation group according to the random number table method,52 cases in each.Both groups were treated with laparoscopic ovarian cystectomy.The control group was treated with bipolar coagulation hemostasis,while the observation group was treated with suture hemostasis.Both groups were followed up for 1 month.The levels of operation-related indexes levels(operation time,intraoperative blood loss,first out-of-bed time and hospitalization time),the ovarian function indexes levels[peak systolic velocity of ovarian artery(PSV),basal antral follicle count(AFC)],and sex hormone indexes levels[follicle stimulating hormone(FSH),luteinizing hormone(LH),estradiol(E_(2))]before and after the surgery,and the incidence of complications were compared between the two groups.Results:The operation time of the observation group was longer than that of the control group,the intraoperative blood loss was more than that of the control group,and the differences were statistically significant(P<0.05).1 month after the surgery,the levels of PSV,AFC and E2 in the two groups were lower than those before the surgery,but those in the observation group were higher than those in the control group;the levels of FSH and LH in the two groups were higher than those before the surgery,but those in the observation group were lower than those in the control group;and the differences were statistically significant(P<0.05).However,there were no significant differences in the first out-of-bed time,the hospitalization time and the incidence of complications between the two groups(P>0.05).Conclusions:Suture hemostasis applied to the patients undergoing laparoscopic ovarian cystectomy can improve the levels of ovarian funct
