腹腔镜腹股沟疝修补术手术入路选择对临床治疗效果影响

摘　　要：目的:分析腹腔镜腹股沟疝修补术手术入路选择对临床治疗效果的影响。方法:选取江西省中西医结合医院2021年1月—2022年1月收治的腹股沟疝且选择进行腹腔镜疝气修补术治疗患者66例,随机均分为对照组33例、试验组33例。对照组应用腹腔镜前方入路行疝气修补术治疗,试验组应用腹腔镜腹膜外入路行疝气修补术治疗。对比分析两组手术相关指标、炎症因子水平及并发症发生率。结果:试验组手术时间、术中出血量、术后下床活动时间、住院时间均明显优于对照组,差异均有统计学意义(P<0.05)。术前,TNF-α、IL-6水平组间比较,差异均无统计学意义(P>0.05);术后试验组的TNF-α、IL-6水平均低于对照组,差异均有统计学意义(P<0.05)。试验组并发症发生率为6.06%(2/33),略低于对照组的15.15%(5/33),差异无统计学意义(P>0.05)。结论:针对腹股沟疝患者,应用腹膜外入路行腹腔镜疝气修补术可有效降低手术创伤,缩短患者术后下床活动时间及住院时间,减轻炎症反应,同时并发症发生率较低。Objective:To analyze the influence of surgical approach selection on clinical treatment effect of laparoscopic inguinal hernia repair.Method:Sixty-six patients with inguinal hernia in Jiangxi Hospital of Integrated Traditional Chinese and Western Medicine from January 2021 to January 2022 were selected for laparoscopic hernia repair.They were randomly divided into the control group(33 cases)and the experimental group(33 cases).The control group underwent hernia repair through anterior laparoscopic approach,and the experimental group underwent hernia repair through external peritoneal laparoscopic approach.Operation-related indexes,levels of inflammatory factors and incidence of complications were compared between the two groups.Result:The operation time,intraoperative blood loss,postoperative ambulation time,and hospital stay time in the experimental group were significantly better than those in the control group,the differences were statistically significant(P<0.05).There were no significant differences in TNF-αand IL-6 levels between the groups before operation(P>0.05),and the levels of TNF-αand IL-6 in experimental group after operation were lower than those in the control group(P<0.05).The complication rate in the experimental group was 6.06%(2/33),which was slightly lower than 15.15%(5/33)in the control group,the difference was not statistically significant(P>0.05).Conclusion:For patients with inguinal hernia,laparoscopic hernia repair using an extraperitoneal approach can effectively reduce surgical trauma,shorten the postoperative time to get out of bed and time of hospital stay,reduce inflammation reaction,the incidence of complications is lower.
