养血祛风方外洗联合加巴喷丁胶囊治疗尿毒症皮肤瘙痒的效果

摘　　要：目的:观察养血祛风方外洗联合加巴喷丁胶囊治疗尿毒症皮肤瘙痒的效果。方法:选取2022年6月—2023年5月在景德镇市中医医院接受治疗的尿毒症皮肤瘙痒患者60例,按随机数字表法分为常规组和中医组。常规组(30例)给予加巴喷丁胶囊治疗,中医组(30例)在常规组基础上加用养血祛风方外洗治疗。比较两组治疗效果、不同时间的瘙痒程度、实验室指标及不良反应。结果:中医组总有效率为96.67%,高于常规组的73.33%,差异有统计学意义(P<0.05)。治疗2、4周后,两组四项目瘙问卷(FIIQ)评分均较治疗前降低,中医组FIIQ评分均低于常规组,差异均有统计学意义(P<0.05)。治疗4周后,两组白细胞介素-6(IL-6)、血磷、甲状旁腺激素(PTH)均低于治疗前,且中医组均低于常规组,差异均有统计学意义(P<0.05)。两组的不良反应发生率比较,差异无统计学意义(P>0.05)。结论:加巴喷丁胶囊联合养血祛风方外洗可有效缓解尿毒症患者皮肤瘙痒程度,改善炎症反应,治疗效果更佳且安全性良好。Objective:To observe the curative effect of Yangxue Qufeng Decoction external wash combined with Gabapentin Capsules in the treatment of uraemic pruritus.Method:A total of 60 patients with uremic pruritus treated in Jingdezhen Hospital of Traditional Chinese Medicine from June 2022 to May 2023 were selected and divided into the conventional group and the traditional Chinese medicine group according to random number table method.The conventional group(30 cases)was treated with Gabapentin Capsules,and the traditional Chinese medicine group(30 cases)was treated with Yangxue Qufeng Decoction external wash on the basis of the conventional group.The therapeutic effect,itching degree at different time,laboratory indexes and adverse reactions were compared between the two groups.Result:The total effective rate of the traditional Chinese medicine group was 96.67%,which was higher than 73.33%of the conventional group,the difference was statistically significant(P<0.05).After 2 and 4 weeks of treatment,the four-item itch questionnaire(FIIQ)scores of the two groups were lower than those before treatment,and FIIQ scores in the traditional Chinese medicine group were lower than those in the conventional group,the differences were statistically significant(P<0.05).After 4 weeks of treatment,the interleukin-6(IL-6),serum phosphorus and parathyroid hormone(PTH)in the two groups were lower than those before treatment,and those in the traditional Chinese medicine group were lower than those in the conventional group,the differences were statistically significant(P<0.05).There was no significant difference in the incidence of adverse reaction between the two groups(P>0.05).Conclusion:Gabapentin Capsules combined with Yangxue Qufeng Decoction external wash can effectively relieve the degree of pruritus in uremia patients,improve the inflammatory response,and has better therapeutic effect and good safety.
