摘　　要：目的 探讨采取颈后路单开门椎管成型术联合神经根管扩大术治疗脊髓型颈椎病的临床效果,为临床提供参考。方法 按随机数字表法将2019年1月至2023年1月南通瑞慈医院收治的80例脊髓型颈椎病患者分为对照组和观察组,各40例。对照组患者给予颈后路单开门椎管成型术治疗,观察组患者在对照组基础上给予神经根管扩大术治疗。比较两组患者临床疗效、术后相关指标、视觉模拟量表(VAS)疼痛评分及术后并发症发生情况。结果 观察组患者的整体疗效优于对照组,且总有效率高于对照组(P<0.05)。观察组患者脊髓后移距离大于对照组,日本骨科协会(JOA)评分改善率高于对照组(P<0.05)。两组患者术后3个月的VAS疼痛评分低于术后1个月和术前,术后1个月的VAS疼痛评分低于术前,且观察组低于对照组(P<0.05)。两组患者术后并发症总发生率比较,差异无统计学意义(P>0.05)。结论 采取颈后路单开门椎管成型术与神经根管扩大术联合治疗脊髓型颈椎病的临床效果显著,可有效解除脊髓、神经根压迫,并改善患者神经功能和疼痛症状,安全性理想。Objective To explore the clinical effect of posterior cervical single door laminoplasty combined with nerve root canal enlargement in the treatment of cervical spondylotic myelopathy,and provide reference for clinical practice.Methods 80 patients with cervical spondylotic myelopathy admitted to Nantong Ruici Hospital from January 2019 to January 2023 were selected as study subjects,divided into control group and observation group by random number table method,with 40 patients included in each group.Patients in the control group were treated with posterior cervical single door laminoplasty,while patients in the observation group were treated with nerve root canal enlargement on the basis of the control group.The clinical efficacy,postoperative related indicators,scores of pain evaluated by visual analogue scale(VAS)and occurrence of the complications between two groups of patients were compared.Results The overall efficacy of the observation group was better than that of the control group,and the total effective rate was higher than that of the control group(P<0.05).The spinal cord displacement distance of the observation group patients were greater than that of the control group,and the improvement rate of the Japanese orthopaedic association(JOA)score was higher than that of the control group(P<0.05).The scores of pain evaluated by VAS of the two groups of patients at 3 months after surgery were lower than those at 1 month after surgery and before surgery,and the scores at 1 month after surgery were lower than that before surgery,and the score of the observation group was lower than the control group(P<0.05).There was no significant difference in the total incidence of postoperative complications between the two groups of patients(P>0.05).Conclusion The treatment of posterior cervical single door laminoplasty combined with nerve root canal enlargement for cervical spondylotic myelopathy has a significant clinical effect.It can effectively relieve spinal cord and nerve root compression,improve neurological fu
