摘　　要：目的:观察手动负压吸宫术(MVA)后搔刮宫腔对手术结局的影响。方法:在山东省妇幼保健院、四川省眉山蕴缨妇产医院、广东省韶关市妇幼保健院、山东省德州市妇幼保健院、北京协和医院等5家医疗机构选择≤25岁自愿要求终止8周内妊娠的473例青少年女性作为观察对象,于术前以随机表法分为搔刮组和不搔刮组,比较两组流产后阴道出血情况、月经恢复时间及流产并发症等。结果:搔刮组和不搔刮组纳入效果分析者分别为234例和235例,术后1个月随访时,流产后阴道出血多和其他异常出血的主诉率,搔刮组分别为2.1%和4.7%,不搔刮组分别为4.2%和5.5%,月经恢复者占比搔刮组为87.2%(204例),不搔刮组为91.5%(215例)。月经恢复的时间搔刮组(36.2±7.2)d,不搔刮组(36.0±7.9)d。术后并发症,搔刮组不全流产2例(0.9%),不搔刮组术后急性盆腔炎1例(0.4%)。上述指标两组间差异均无统计学意义(P>0.05)。结论:对于≤25岁青少年女性8周内妊娠,MVA后不进行宫腔搔刮对手术结局无不利影响。Objective:To observe the effects of uterine curettage after manual vacuum aspiration(MVA)of female adolescents on their surgical outcomes.Methods:473female adolescents≤25years old who voluntarily requested to terminate pregnancy within 8gestational weeks were selected as the study subjects from 5medical institutions.These adolescents before surgery were randomly divided into curettage group and non-curettage group by random table method.The vaginal bleeding after abortion,the menstrual recovery time and the abortion complications situation of the adolescents were compared between the two groups.Results:There were 234cases in the curettage group and 235cases in the non-curettage group.In 1month follow-up after surgery,the chief complaint ratios of more vaginal bleeding and other abnormal bleeding of the adolescents in the curettage group were 2.1%and 4.7%,and which of the adolescents in the non-curettage group were 4.2%and 5.5%.The ratio of restoration of menstruation of the adolescents in the curettage group was 87.2%(204cases),and which of the adolescents in the non-curettage group was 91.5%(215cases).The menstrual recovery time of the adolescents in the two groups were 36.2±7.2days and 36.0±7.9days.As for the postoperative complications,there were 2(0.9%)cases with incomplete abortion in the curettage group,and 1(0.4%)case with acute pelvic inflammatory disease in the non-curettage group,and the postoperative complications situations of the adolescents had no significant differences between the two groups(P>0.05).Conclusion:For the female adolescents≤25years old who are pregnant within 8gestational weeks,the adolescents without uterine curettage after abortion has no adverse effects on their surgical outcomes.
