桂枝加龙骨牡蛎汤治疗手汗症体会及其方证探析

摘　　要：桂枝加龙骨牡蛎汤在《金匮要略》中原治虚劳失精。张红珍教授取其调和营卫、温心阳、敛津液之意,并结合《难经》“损其心者,调其营卫”的记载,用治营卫失和型原发性手汗症,疗效确切。由此引发对桂枝加龙骨牡蛎汤方证的新思考,认为其根本病机在于营卫失和,方中以桂枝汤为基础调和营卫,使人体阴阳和合、气血以顺,加龙骨、牡蛎宁心神、敛津液,可治疗包含自汗盗汗、不寐、惊悸、遗精等诸多病症。The combination of Guizhi and Longgu Muli Decoction is originally used to treat over-strain and loss of essence in Synopsis of the Golden Chamber.Professor Zhang Hongzhen takes the meaning of regulating nutritive Qi and defensive Qi,warming heart yang and astringing body fluid,combines with the record in the Classic of Difficults that“the impairment of heart can be cured with regulating nutritive Qi and defensive Qi”,and treats the primary palmar hyperhidrosis due to disharmony of nutritive Qi and defensive Qi.The curative effect is definite.This leads to new thinking on the combination of Guizhi and Longgu Muli Decoction.It is believed that the basic pathogenesis lies in the disharmony of nutritive Qi and defensive Qi.The formula is based on Guizhi Decoction to regulate nutritive Qi and defensive Qi,resulting in the harmonization of Yin and Yang and smooth flow of Qi and blood.In the formula,Longgu and Muli are added to calm heart spirit and astringe body fluid,which can treat a number of diseases including spontaneous sweating,night sweating,insomnia,palpitations,spermatorrhea and so on.
