摘 要:目的探讨2型糖尿病(T2DM)伴微血管病变患者C肽、胰岛素抵抗水平与凝血功能的相关性。方法选取2021年6月至2023年5月该院收治的T2DM患者96例,根据是否合并微血管病变分为合并组(n=51)与未合并组(n=45)。所有患者均接受C肽、胰岛素抵抗水平与凝血功能相关指标水平检测,统计两组胰岛素抵抗指数、C肽、凝血功能相关指标水平,分析胰岛素抵抗指数、C肽水平与凝血功能相关指标水平的关联性。结果合并组胰岛素抵抗指数高于未合并组,C肽水平低于未合并组,差异有统计学意义(P<0.05)。合并组纤维蛋白原水平高于未合并组,凝血酶时间、活化部分凝血活酶时间、凝血酶原时间短于未合并组,差异有统计学意义(P<0.05)。胰岛素抵抗指数与纤维蛋白原呈正相关关系,与凝血酶时间、活化部分凝血活酶时间、凝血酶原时间呈负相关关系(P<0.05),C肽与纤维蛋白原呈负相关关系,与凝血酶时间、活化部分凝血活酶时间、凝血酶原时间呈正相关关系(P<0.05)。结论T2DM伴微血管病变患者C肽水平下降,胰岛素抵抗水平增高,且与凝血功能存在显著关联性。Objective To investigate the correlation between C-peptide,insulin resistance and coagulation function in type 2 diabetes mellitus(T2DM)patients with microangiopathy.Methods Ninety six patients with T2DM admitted to Jingyang district traditional chinese medicine hospital from June 2021 to May 2023 were selected and divided into a combined group(n=51)and an unmerged group(n=45)based on whether they had microvascular lesions.All patients underwent C-peptide,insulin resistance levels,and coagulation related indicators testing.The levels of insulin resistance index,C-peptide,and coagulation related indicators in both groups were statistically analyzed,and the correlation between insulin resistance index,C-peptide levels,and coagulation related indicators was analyzed.Results The insulin resistance index of the combined group was higher than that of the non combined group,and the C-peptide level was lower than that of the non combined group,and the differences were statistically significant(P<0.05).The fibrinogen level in the combined group was higher than that in the non combined group,and the thrombin time,activated partial thromboplastin time,and prothrombin time were shorter than those in the non combined group,and the differences were statistically significant(P<0.05).The insulin resistance index is positively correlated with fibrinogen,and negatively correlated with thrombin time,activated partial thromboplastin time,and prothrombin time(P<0.05).C-peptide is negatively correlated with fibrinogen,and positively correlated with thrombin time,activated partial thromboplastin time,and prothrombin time(P<0.05).Conclusion Patients with T2DM and microvascular disease have a decrease in C-peptide levels,an increase in insulin resistance levels,and a significant correlation with coagulation function.
