摘　　要：目的:探讨肌肉骨骼超声图像特征与风湿疾病的相关性。方法:选取2020年12月~2022年12月本院收治的80例风湿疾病患者为观察对象,对所有患者通过肌肉骨骼超声检查,分析肌肉骨骼超声图像特征与风湿疾病的相关性。结果:通过肌肉骨骼超声图像检查发现,炎性关节病患者45例,其中包括类风湿关节炎患者25例,脊柱关节炎及其相关的关节炎11例,痛风关节炎9例,系统性硬化症患者21例,超声出现回声强度异常增强现象,风湿性多肌痛患者14例,不同类型的风湿疾病显示出不同的超声图像特征;不同风湿疾病患者超声总评分、积液评分对比无显著差异(P>0.05),不同风湿疾病患者的滑膜血流评分、骨侵蚀评分、滑膜厚度评分对比差异显著(P<0.05);Spearman相关分析结果显示:超声总评分、积液评分与风湿疾病无明显相关性(P>0.05),滑膜血流评分、骨侵蚀评分、滑膜厚度评分与风湿疾病具有明显相关性(P<0.05)。结论:风湿类疾病包括风湿性多肌痛、系统性硬化症、痛风性关节炎、脊柱关节炎和风湿性关节炎,肌肉骨骼超声主要扫描位置表浅的肌肉与关节,能够早期发现骨骼和肌肉系统中出现的炎性反应,具有可重复性、敏感性高、安全性高的优点,其图像特征与风湿类疾病具有明显相关性。Objective:To explore the correlation between musculoskeletal ultrasound image features and rheumatic diseases.Methods:A total of 80 patients with rheumatic diseases admitted to our hospital from December 2020 to December 2022 were selected.Musculoskeletal ultrasound examination was performed on all patients to analyze the correlation between musculoskeletal ultrasound image features and rheumatic diseases.Results:Through musculoskeletal ultrasound image examination,45 patients with inflammatory arthritis were found,including 25 patients with RA,11 patients with spondyloarthritis and its related arthritis,9 patients with gout arthritis,21 patients with systemic sclerosis,abnormal echo intensity enhancement phenomenon appeared in ultrasound,14 patients with rheumatic polymyalgia,different types of rheumatic diseases showed different ultrasound image characteristics There was no significant difference in the total ultrasound score and effusion score of patients with different rheumatic diseases(P>0.05),and the significant difference in synovial blood flow score,bone erosion score and synovial thickness score of patients with different rheumatic diseases was significant(P<0.05).Spearman correlation analysis showed that total ultrasound score and effusion score were not significantly correlated with rheumatic disease(P>0.05),but synovial blood flow score,bone erosion score and synovial thickness score were significantly correlated with rheumatic disease(P<0.05).Conclusion:Rheumatic diseases include rheumatic polymyalgia,systemic sclerosis,gouty arthritis,spondyloarthritis and rheumatoid arthritis.Musculoskeletal ultrasound mainly scans the superficial muscles and joints,which can early detect the inflammatory reaction in the bone and muscle system.It has the advantages of repeatability,high sensitivity,and high safety.Its image features are significantly correlated with rheumatic diseases.
