干式免疫分析仪检测血清C-反应蛋白、白细胞介素-6、降钙素原在老年细菌性与病毒性上呼吸道感染鉴别诊断中的应用价值

摘　　要：目的:探讨与分析干式免疫分析仪测定下血清C-反应蛋白(CRP)、白细胞介素-6(IL-6)、降钙素原(PCT)联合检测在老年细菌性与病毒性上呼吸道感染中的差异诊断价值。方法:选择本院老年细菌性、病毒性上呼吸道感染患者22例(细菌组)、61例(病毒组),检测两组血清CRP、IL-6、PCT含量,判断鉴别诊断的价值。结果:病毒组血清CRP、IL-6、PCT含量低于细菌组(P<0.05)。多因素二元Logistic回归分析显示老年病毒性上呼吸道感染都为影响血清CRP、IL-6、PCT含量的重要危险因素(P<0.05)。受试者操作特征曲线分析显示血清CRP、IL-6、PCT与联合检测在老年细菌性与病毒性上呼吸道感染中具有诊断价值。结论:老年病毒性上呼吸道感染患者多存在血清CRP、IL-6、PCT的低表达,血清CRP、IL-6、PCT联合检测对鉴别诊断老年细菌性与病毒性上呼吸道感染具有重要价值。Objective:To explore and analysis the differential diagnostic value of combined detection of serum C-reaction protein(CRP),interleukin-6(IL-6),and calcitonin(PCT)in bacterial and viral upper respiratory infection in the elderly by dry immune analyzer.Methods:Choose 22 cases(bacterial group)and 61 cases(viral group)of elderly patients with bacterial and viral upper respiratory tract infections in our hospital,test the serum CRP,IL-6 and PCT levels of the two groups to determine the value of differential diagnosis.Results:Serum CRP,IL-6,and PCT levels were lower in the viral group than in the bacterial group(P<0.05).Multifactorial binary Logistic regression analysis showed that all viral upper respiratory tract infections in the elderly were significant risk factors affecting serum CRP,IL-6,and PCT levels(P<0.05).ROC curve analysis reveals the diagnostic value of serum CRP,IL-6,PCT with combined tests in bacterial and viral upper respiratory tract infections in elderly people.Conclusion:Elderly patients with viral upper respiratory tract infections often have lower expression of serum CRP,IL-6,PCT.The combined detection of serum CRP,IL-6,and PCT is of great value in the differential diagnosis of bacterial and viral upper respiratory tract infections in the elderly.
