血常规主要指标检测中脂血对Sysmex XN-1000血液分析仪测定结果的影响

摘　　要：目的:采用Sysmex XN-1000血液分析仪对血常规主要指标进行测定,分析脂血对其结果所产生的影响。方法:选取没有溶血、脂血与黄疸的标本20份(当作对照组),对其实施离心操作,将上层血浆成分给予去除,将Sysmex XN-1000血液分析仪原装配套稀释液(同体积)加入,对血常规进行测定(试验组),把最终结果对比于对照组;选取没有脂血、黄疸、溶血且已经测定的血常规标本66份,将其随机分成三组,用贝克曼AU5800全自动生化分析仪对甘油三酯(TG)进行测定,依据TG含量的差异对脂血程度实施分级:轻度脂血组(没有加脂肪乳剂的测定结果当作对照1组)、中度脂血组(没有加脂肪乳剂的测定结果当作对照2组)与重度脂肪组(没有加脂肪乳剂的测定结果当作对照3组);把测定结果均对比于没有加脂肪乳剂者的测定结果。结果:对照1组较之轻度脂血组,13项参数结果对比并无显著差异(P>0.05);中度脂血组较之对照2组,以及重度脂血组相比对照3组,白细胞(WBC)、平均红细胞血红蛋白浓度(MCHC)、平均红细胞血红蛋白含量(MCH)与血红蛋白(Hb)都有明显升高(P<0.05)。结论:在采用Sysmex XN-1000血液分析仪对血常规指标进行检测时,中、重度脂血对WBC、MCH、Hb、MCHC参数有着较大影响,且伴随脂血程度的加重,其影响也随之加大。Objective:To use the Sysmex XN-1000 blood analyzer to measure the main blood routine indexes and analyze the influence of blood fat on the results.Methods:Select 20 specimens without hemolysis,lipomatosis,and jaundice(as the control group),perform centrifugation on them,remove the upper plasma components,add the original matching diluent(same volume)of Sysmex XN-1000 blood analyzer,and measure the blood routine(experimental group).Compare the final results with the control group;Select 66 blood routine standards that have been tested and are free of fatty blood,jaundice,and hemolysis,and randomly divide them into three groups.Use the Beckman AU5800 automatic biochemical analyzer to measure triglycerides(TG),and grade the degree of fatty blood based on the difference in TG content:mild fatty blood group(the test results without fat emulsion are used as control group 1)The moderate hyperlipidemia group(the test results without fat emulsion were treated as control group 2)and the severe hyperlipidemia group(the test results without fat emulsion were treated as control group 3);Compare the measurement results with those of those without added fat emulsion.Results:There was no significant difference in the results of 13 parameters between the control group 1 and the mild lipid blood group(P>0.05);Compared with the control group 2 and the severe lipidemia group 3,the moderate lipidemia group showed significant increases in white blood cell(WBC),mean corpuscular hemoglobin concentration(MCHC),mean corpuscular hemoglobin content(MCH),and hemoglobin(Hb)(P<0.05).Conclusion:When using Sysmex XN-1000 blood analyzer to detect blood routine parameters,moderate and severe blood fat has a great impact on WBC,MCH,Hb,MCHC parameters,and the impact will also increase with the aggravation of blood fat.
