胰岛素泵联合动态血糖检测在肿瘤合并糖尿病患者中的应用

摘　　要：目的:评价胰岛素泵联合动态血糖检测在肿瘤合并糖尿病患者中的应用。方法:选择2020年6月~2022年5月在本院进行化疗的肿瘤合并糖尿病患者596例。患者随机分为胰岛素泵联合动态血糖检测治疗组(联合组,n=298)与对照组(n=298)。对照组患者给予门冬胰岛素注射治疗,联合组采用胰岛素泵联合动态血糖监测系统治疗。比较治疗前后对照组、联合组血糖指标与血脂指标,比较治疗后两组低血糖发病率、胰岛素用量和血糖达标时间。结果:联合组治疗后糖化血红蛋白、2h餐后血糖、空腹血糖均较对照组显著降低(P<0.05)。联合组治疗后低密度脂蛋白胆固醇、总胆固醇、甘油三酯均较对照组同期值显著降低,高密度脂蛋白胆固醇较对照组同期值显著提高(P<0.05)。联合组治疗后低血糖发病率、胰岛素用量、血糖达标时间均较对照组显著降低(P<0.05)。结论:采用胰岛素泵联合动态血糖检测能够实现对肿瘤合并糖尿病患者的个性化降糖治疗,治疗后患者低血糖发病率更低、血糖控制效果更好,治疗的安全性更高。Objective:To evaluate the application of insulin pump combined with ambulatory glucose testing in patients with oncology combined with diabetes mellitus.Methods:596 patients with oncology combined with diabetes mellitus who underwent chemotherapy in our hospital from June 2020 to May 2022 were included.Patients were randomly divided into insulin pump combined with dynamic glucose testing treatment group(combined group,n=298)and control group(n=298).Patients in the control group were treated with menthol insulin injection,while the combined group was treated with an insulin pump combined with a dynamic blood glucose monitoring system.The glycemic indexes and lipid indexes of the control group and the combined group were compared before and after treatment,and the incidence of hypoglycemia,insulin dosage and time to achieve the blood glucose standard were compared between the two groups after treatment.Results:Glycosylated haemoglobin,2h postprandial glucose and fasting glucose were significantly lower in the combined group after treatment compared with the control group(P<0.05).LDL cholesterol,total cholesterol and triglycerides were significantly lower and HDL cholesterol was significantly higher in the combined group than in the control group(P<0.05).The incidence of hypoglycemia,insulin dosage and time to blood glucose standard were all significantly lower in the combined group after treatment compared with the control group(P<0.05).Conclusion:The use of insulin pump combined with dynamic blood glucose testing can achieve personalized glucose-lowering treatment for patients with oncology combined with diabetes mellitus.
