摘  要:目的:探讨彩超诊断仪超声剪切波弹性成像(SWE)对甲状腺良恶性结节的诊断价值。方法:选取2021年1月~2022年4月在本院行手术或穿刺活检病理证实的甲状腺结节患者78例(共96个结节),所有受检者均接受SWE检查,以病理结果为金标准,比较甲状腺良恶性结节弹性模量值的差异,并采用ROC曲线分析SWE弹性模量值诊断甲状腺良恶性结节的效能。结果:纳入的78例患者共96个结节中,直径5~40mm,平均(12.0±6.0)mm,其中良性结节62个,占比64.58%,恶性结节34个,占比35.42%。恶性结节组弹性模量值Emax、Emean、Emin及Esd水平均高于良性结节组,差异有统计学意义(P<0.05)。ROC曲线分析显示,弹性模量值Emax、Emean、Emin及Esd预测甲状腺恶性结节的曲线下面积(AUC)分别为0.939、0.770、0.710、0.870,预测敏感度为97.1%、61.8%、73.5%、73.6%,特异度为83.9%、74.2%、59.7%、85.5%。结论:SWE能够独立检测甲状腺结节的杨氏模量值,从而有助于其良恶性病的鉴别诊断,其中Emax诊断效能最高,可作为甲状腺结节诊断的一种有效方法。Objective:To investigate the diagnostic value of ultrasonic shear wave elastography(SWE)in benign and malignant thyroid nodules.Methods:78 patients with thyroid nodules confirmed by surgery or biopsy pathology in our hospital from January 2021 to April 2022 were selected(a total of 96 nodules).All subjects underwent SWE examination.The pathological results were used as the gold standard to compare the difference of elastic modulus between benign and malignant thyroid nodules.ROC curve was used to analyze the efficacy of SWE elastic modulus in the diagnosis of benign and malignant thyroid nodules.Results:A total of 96 nodules were included in 78 patients,the diameter was 5-40mm,with an average of(12.0±6.0)mm.There were 62 benign nodules,accounting for 64.58%,and 34 malignant nodules,accounting for 35.42%.The elastic modulus values of Emax,Emean,Emin and Esd in the malignant nodules group were higher than those in the benign nodules group,and the differences were statistically significant(P<0.05).ROC curve analysis showed that the area under the curve(AUC)of elastic modulus values Emax,Emean,Emin and Esd to predict malignant thyroid nodules were 0.939,0.770,0.710 and 0.870,respectively,and the predictive sensitivity was 97.1%,61.8%,73.5%and 73.6%.The specificity was 83.9%,74.2%,59.7%,85.5%.Conclusions:SWE can independently detect the Young’s modulus of thyroid nodules in real time,which is helpful for the differential diagnosis of benign and malignant thyroid nodules.Emax has the highest diagnostic efficiency and can be used as an effective method for the diagnosis of thyroid nodules.
