贞莲明目胶囊治疗肝肾阴虚痰瘀湿阻型非增殖期糖尿病视网膜病变的临床研究

摘　　要：目的:观察贞莲明目胶囊对肝肾阴虚痰瘀湿阻型非增殖期糖尿病视网膜病变(Nonproliferative Diabetic Retinopathy,NPDR)治疗效果。方法:随机选取河北省沧州中西医结合医院内分泌科就诊的90例中医辨证为肝肾阴虚痰瘀湿阻型的NPDR患者,随机分为对照组、试验1组、试验2组,各组30例。三组在常规治疗基础上分别给予羟苯磺酸钙分散片、贞莲明目胶囊、羟苯磺酸钙分散片联合贞莲明目胶囊治疗3个月。比较治疗前后三组患者的治疗效果,包括视野灰度值、眼底积分、血管瘤体积、出血斑面积、黄斑厚度、中医证候积分。结果:与治疗前相比,三组均能显著降低视野灰度值、眼底积分、血管瘤体积、出血斑面积、黄斑厚度、中医证候积分,差异具有统计学意义(P<0.05)。治疗后三组进行组间比较,与对照组相比,试验1组和试验2组能进一步降低视野灰度值、眼底积分、血管瘤体积、出血斑面积、黄斑厚度、中医证候积分,差异具有统计学意义(P<0.05)。治疗后试验1组与试验2组眼底积分比较,差异无统计学意义(P>0.05),中医证候积分、视野灰度值、血管瘤体积、出血斑面积、黄斑厚度比较,差异具有统计学意义(P<0.05)。治疗后总有效率比较,试验1组与试验2组相比差异无统计学意义(P>0.05),二者与对照组相比,差异具有统计学意义(P<0.05)。结论:贞莲明目胶囊治疗肝肾阴虚痰瘀湿阻型NPDP变效果显著,优于单纯应用羟苯磺酸钙,能够显著改善中医证候积分、眼底积分,降低视野灰度值、血管瘤体积、出血斑面积、黄斑厚度,延缓视网膜病变的进展。Objective:To observe the therapeutic effect of the Zhenlian Mingmu capsules(贞莲明目胶囊)on non-proliferative diabetic retinopathy(NPDR)of deficiency of Yin(阴),phlegm stasis and dampness of liver and kidney.Methods:There were 90 cases of NPDR with syndrome differentiation of Yin deficiency of liver and kidney and phlegm stasis and dampness selected from endocrinology department of hospital and divided into the control group,the test group 1 and the test group 2,with 30 cases in each group.On the basis of conventional treatment,the three groups were respectively given calcium oxybenzenesulfate dispersing tablets,the Zhenlian Mingmu capsules,calcium oxybenzenesulfate dispersing tablets combined with the Zhenlian Mingmu capsules for 3 months.The therapeutic effects in the three groups before and after treatment were compared,including visual field gray value,fundus integral,hemangioma volume,blood spot area,macular thickness and TCM symptom integral.Results:Compared with before treatment,Clinical Journal of Chinese Medicine 2023 Vol.(15)No.31 grayscale value of visual field,fundus score,volume of hemangioma,area of blood spot,thickness of macular and TCM symptom score in three groups were significantly reduced,with statistical significance differences(P<0.05).After treatment,those in the three groups were compared.Compared with the control group,the grayscale value of visual field,fundus score,hemangioma volume,blood spot area,macular thickness and TCM syndrome score in test group 1 and te group 2 were further reduced,and the differences were statistically significant(P<0.05).After treatment,there was no significant difference in fundus scores between test group 1 and test group 2(P>0.05),but there were statistically significant differences in TCM symptom scores,visual field gray value,hemangioma volume,blood spot area and macular thickness between test the group 1 and the test group 2(P<0.05).After treatment,there was no significant difference in the total effective rate between the group 1 and the group 2(
