基于数据挖掘探讨中药复方治疗寒湿型膝骨关节炎的用药规律

摘　　要：目的:利用数据挖掘技术探讨寒湿型膝骨关节炎的中药复方用药规律和组方特点,为临床治疗提供思路。方法:检索中国知识基础设施数据库、万方数据库、维普数据库、中国生物医学文献数据库建库至2022年9月1日收录的中药复方治疗寒湿型膝骨关节炎的临床研究;利用中医传承辅助平台建立寒湿型膝骨关节炎中药处方数据库;对数据进行描述性分析、关联规则分析、聚类分析。结果:共纳入文献135篇,最终纳入文献18篇,涉及中药75味。其中牛膝、独活、鸡血藤为使用频次前三的药物。通过关联规则分析得出最常用的药物组合是牛膝-独活。经辅助平台熵层次聚类分析法发现关联系数较高的核心药对16个,潜在新处方8个。结论:中药复方治疗寒湿型膝骨关节炎的方剂多以祛风除湿为主,兼以补益肝肾、活血止痛为辅。挖掘到的新处方为临床上治疗寒湿型膝骨关节炎选方遣药、提高疗效提供了参考。Objective:To explore the medication law and composition characteristics of TCM medicine compound prescription for cold dampness type knee osteoarthritis by using data mining technology,so as to provide ideas for clinical treatment.Methods:The clinical studies on the treatment of cold dampness type knee osteoarthritis with TCM medicine compound were retrieved from the establishment of CNKI,CSPD,CCD and CBM until September 1,2022.The traditional Chinese medicine prescription database of cold dampness type knee osteoarthritis was established by using the traditional Chinese Medicine Inheritance auxiliary platform.Descriptive analysis,association rule analysis and cluster analysis were performed on the data.A total of 135 literatures were included,and finally 18 literatures were included,involving 75 flavors of TCM medicine.Among them,Niuxi(Radix Achyranthis Bidentatae),Duhuo(Radix Angelicae Pubescentis),Jixueteng(Caulis Spatholobi)were the top three drugs used frequently.Through the analysis of association rules,the most commonly used drug combination is Niuxi-Duhuo.The auxiliary platform entropy hierarchical cluster analysis found 16 core drug pairs with high correlation coefficient and 8 potential new prescriptions.Conclusion:The prescriptions of TCM medicine in the treatment of cold dampness type knee osteoarthritis mainly focus on dispelling wind and dampness,supplemented by Tonifying the liver and kidney,promoting blood circulation and relieving pain.The excavated new prescription provides a reference for the clinical treatment of cold dampness type knee osteoarthritis.
