摘 要:目的:探讨中药复方肠润方影响功能性便秘大鼠结肠运动的作用机制。方法:选取健康SD大鼠72只,随机分为空白组、模型组、麻仁丸组、肠润方高剂量组、肠润方中剂量组、肠润方低剂量组,每组12只。除空白组外,其余各组均采用复方地芬诺脂灌胃建立功能性便秘大鼠肠动力障碍模型。采用多导生理记录仪检测肠润方水提液对大鼠离体结肠张力、收缩波振幅的影响;通过Western-Blot检测大鼠结肠酪氨酸蛋白激酶生长因子受体(Tyrosine Kinase Growth Factor Receptor,C-Kit)、干细胞因子(Stem Cell Factor,SCF)、超极化激活环核苷酸门控的阳离子通道1(Hyperpolarization-activated Cyclic nucleotide-gated Cation Channel 1,HCN1)、超极化激活环核苷酸门控的阳离子通道2(Hyperpolarization-activated Cyclic nucleotide-gated Cation Channel 2,HCN2)的蛋白表达水平;通过RT-PCR实验检测大鼠结肠C-Kit、SCF、HCN1、HCN2的mRNA表达水平。结果:①离体肠管运动实验结果显示,不同浓度肠润方水提液均可改善地芬诺酯所致的大鼠离体结肠张力的下降,同时提高收缩波振幅;②与空白组比较,模型组C-Kit、SCF、HCN1、HCN2在结肠的蛋白及mRNA表达显著降低(P<0.01)。肠润方高剂量组和肠润方中剂量组可显著升高C-Kit、SCF、HCN1、HCN2的蛋白表达及mRNA表达(P<0.01)。结论:肠润方能够显著改善功能性便秘临床症状,其作用机制可能与上调C-Kit、SCF以及HCN通道表达水平,维持Cajal间质细胞(Interstitial Cells of Cajal,ICC)胃肠慢波起搏与传导功能,加强结肠平滑肌收缩,促进结肠运动有关。Objective:To investigate the mechanism of the effect of the TCM medicine compound the Changrun decoction(肠润方)on colon motility in rats with functional constipation(FC).Methods:A total of 72 healthy SD rats were selected and randomly divided into the control group,model group,the Maren pill(麻仁丸)group,the Changrun decoction(high dose,medium dose and low dose)group.Except for the control group,the rest of the groups were treated with the compound difenoxin to establish the FC rat model with intestinal motility disorder.The effect of the Changrun decoction aqueous extract on the tension and contraction wave amplitude of rat isolated colon was measured by multilead physiological recording instrument.The protein expression levels of C-Kit,SCF,HCN1 and HCN2 in rat colon were detected by Western blot.The mRNA expression levels of C-Kit,SCF,HCN1 and HCN2 in rat colon were detected by RT-PCR.Results:①The results of the isolated intestinal experiment showed that the different concentrations of the Changrun decoction aqueous extract could improve the decrease of rat isolated colon tone,which was caused by difenoxin,and increase the contraction wave amplitude.②Compared with the control group,the protein and mRNA expression of C-Kit,SCF,HCN1 and HCN2 in the colon of the model group were significantly decreased(P<0.01).The Changrun decoction(high and medium dose groups)significantly increased the protein expression and mRNA expression of C-Kit,SCF,HCN1 and HCN2(P<0.01).Conclusion:The mechanism behind the significant improvement of clinical symptoms of FC with the Changrun decoction may be related to the upregulation of C-Kit,SCF and HCN channel expression levels and the maintenance of the slow wave pacing and conduction function of ICC,resulting in enhanced contraction of colonic smooth muscle and promotion of colonic motility.
