超声可视化针刀联合火针治疗旁中央型腰椎间盘突出症的临床研究

摘　　要：目的观察旁中央型腰椎间盘突出症(LDH)的超声可视化针刀联合火针治疗的临床效果。方法按随机数字表法将86例LDH患者分为治疗组与对照组各43例。对照组采用火针治疗,治疗组采用超声可视化针刀联合火针治疗,两组均每周治疗1次,共治疗4周。比较两组患者治疗前后疼痛视觉模拟量表(VAS)评分、Oswestry功能障碍指数问卷(ODI)评分及两组超声下多裂肌、腹横肌厚度,血清C反应蛋白(CRP)含量,对临床疗效及安全性进行评价。结果治疗后两组患者VAS评分、ODI评分、血清CRP含量均较治疗前降低(P<0.05),治疗组较对照组评分降低更为显著(P<0.05);两组患者多裂肌、腹横肌厚度较治疗前增厚(P<0.05),治疗组较对照组肌肉增厚更为显著(P<0.05);治疗组总有效率为94.87%,高于对照组的82.50%(P<0.05)。结论超声可视化针刀联合火针治疗可缓解旁中央型LDH疼痛,改善多裂肌、腹横肌肌肉功能,降低血清CRP含量,安全性良好,可很好地发挥治疗旁中央型LDH的作用。Objective:To observe the clinical effect of ultrasonic visualization acupotomy combined with fire needling in the treatment of paracentral lumbar disc herniation(LDH).Methods:A total of 86 patients with LDH were divided into the fire needling group and ultrasonic visualization acupotomy combined with fire needling group according to random number table method,with 43 cases in each group.Both groups were treated once a week for 4 weeks.The visual analogue scale(VAS)scores,Oswestry Disability Index(ODI)scores,the thickness of multifi⁃dus muscle and transversal abdominal muscle under ultrasound,and serum C-reactive protein(CRP)were com⁃pared between the two groups before and after treatment to evaluate the clinical efficacy and safety.Results:After treatment,the VAS and ODI scores,CRP content of the two groups were decreased compared with before treatment(P<0.05),and the scores,CRP content of combination group were significantly decreased compared with the fire needling group(P<0.05).The thickness of multifidus muscle and transversus abdominis muscle in the two groups were thicker than those before treatment(P<0.05),and the muscle thickening in the combined group was more sig⁃nificant than that in the fire needling group(P<0.05).The total effective rate was 94.87%in the combination group,which was higher than 82.50%in the acupuncture group.Conclusion:Ultrasound visualization acupotomy combined with fire needling can relieve the pain of paracentral LDH,improve the multifidus muscle,transversus abdominis muscle activity function,reduce the serum CRP content,and has good safety.It can play a good role in the treatment of paracentral LDH.
