摘  要:目的:应用多模态影像检查总结基底动脉尖综合征影像学表现,为临床提供准确的影像信息,提高基底动脉尖综合征诊治的及时性和精准性,采取有效治疗措施,降低致残率及死亡率,改善预后。方法:回顾性分析2018年6月—2023年7月汕头大学医学院附属粤北人民医院收治的共62例符合基底动脉尖综合征表现患者影像资料,根据患者实际情况,灵活采用头部CT多模态、头部MRI多模态、数字减影血管造影(DSA);回顾性分析基底动脉尖综合征患者的颅脑影像学表现,统计不同影像方法检出该病的阳性率,观察头部CT平扫、头部MRI平扫、弥散加权成像(DWI)组间脑梗死灶部位检出数量间差异。结果:本项研究中发现头部CT平扫基底动脉尖高密度征检出率84%,CTA在识别基底动脉尖栓塞/狭窄方面检出率100%;头部CT平扫、头部MRI平扫、DWI组间脑梗死灶部位检出数量间差异有统计学意义(P<0.05);脑灌注CTP、多模态MRI评估脑缺血/梗死范围更精确;DSA能更准确显示基底动脉尖栓塞/狭窄程度。结论:多模态影像检查明显提高基底动脉尖综合征诊断的准确性,降低漏诊率及误诊率;提供更加全面、准确的信息,有很高的临床应用价值。Objective This study aims to summarize the imaging manifestations of top of the basilar syndrome(TOBS)by using multimodal imaging examinations,providing accurate imaging information for clinical practice to enhance the timeliness and accuracy of diagnosis and treatment of top of the basilar syndrome.Effective treatment measures can be implemented to reduce disability and mortality rates and improve prognosis.Methods Retrospective analysis of imaging data of 62 patients with TOBS manifestations admitted to the Yuebei People's Hospital affiliated with Shantou University School of Medicine from June 2018 to July 2023,based on the individual patient’s condition,flexible utilization of multimodal head CT,multimodal head MRI,and digital subtraction angiography(DSA)were employed.We conducted a retrospective analysis of multimodal imaging examinations on patients with top of the basilar artery syndrome,statistically analyzed the positive rate of detection of the disease by different imaging examinations,and the differences in the number of detected cerebral infarction foci between groups using head CT plain scan,head MRI plain scan,and diffusion-weighted imaging(DWI)were observed.Results In this study,the detection rate of high-density sign at the top of the basilar artery on nonenhanced head CT was 84%,and the detection rate of embolism/stenosis at the top of the basilar artery by CTA was 100%.There was a statistically significant difference in the number of detected cerebral infarction lesions between the non-enhanced head CT scan,the nonenhanced head MRI scan,and diffusion-weighted imaging groups(P<0.05);compared to non-enhanced CT and MRI,cerebral perfusion and multimodal MRI are more accurate in assessing the extent of cerebral ischemia/infarction,and DSA was more accurately display the degree of basilar artery top embolism/stenosis.Conclusion Multimodal imaging examinations significantly enhance the diagnostic accuracy of top of the basilar syndrome,reduces the rate of missed diagnosis and misdiagnosis,and provid
