院内感染菌及药敏谱的统计分析

摘　　要：目的 统计并分析引起院内感染的主要致病菌及致病菌对常用抗菌药物的敏感谱。方法 收集天水市第一人民医院2019年2月-2022年8月的万例院感标本,共分离出病原菌4861株。采用英国先德Aris全自动细菌鉴定/药敏系统,对分离出的致病菌进行细菌鉴定和药敏试验,统计并分析主要院内感染菌和其敏感度高的抗菌药物敏感度种类。结果 共分离出致病菌4861株,其中革兰阴性杆菌3462株、占比71.2%;革兰阳性球菌1399株、占比28.8%。主要革兰阴性杆菌中,大肠埃希菌对常用抗菌药物哌拉西林/舒巴坦、头孢哌酮/舒巴坦、亚胺培南的敏感度高;肺炎克雷伯菌对哌拉西林/舒巴坦、头孢哌酮/舒巴坦、亚胺培南、庆大霉素、环丙沙星、左氧氟沙星的敏感度高;铜绿假单胞菌对头孢吡肟、美洛培南的敏感度高;不动杆菌对常用抗菌药物哌拉西林、头孢他啶、庆大霉素、环丙沙星、SMZco的耐药率高。主要革兰阳性球菌中,金黄色葡萄球菌对常用抗菌药物万古霉素、替考拉宁、利奈唑胺的敏感度高;表皮葡萄球菌对万古霉素、替考拉宁、利奈唑胺、利福平的敏感度高。结论 通过对本院的主要致病菌及致病菌对常用药物的敏感谱检测分析,革兰阳性球菌以葡萄球菌属为主,它们对万古霉素、替考拉宁、利奈唑胺等抗菌药物敏感度高;革兰阴性杆菌以大肠埃希菌等肠杆菌属为主,它们对亚胺培南、头孢哌酮/舒巴坦、哌拉西林/舒巴坦等抗菌药物敏感度高,本次研究对控制院内感染以及对感染患者的诊疗具有重要的指导意义。Objective To explore main pathogenic bacteria causing hospital infections and sensitivity spectrum of pathogenic bacteria to commonly used antibiotics.Methods The paper carried on bacterial identification and drug sensitivity tests on isolated pathogenic bacteria with British Xiande Aris fully automated bacterial identification/drug sensitivity system.Results A total of 4861 strains of pathogenic bacteria were isolated,including 3462 strains of Gram negative bacteria,accounting for 71.2%;1399 strains of Gram positive cocci,accounting for 28.8%.Gram positive cocci were mainly staphylococcus,highly sensitive to vancomycin,teicoplanin,linezolid and other antimicrobial agents.Gram negative bacilli were mainly enterobacter,including escherichia coli,highly sensitive to imipenem,cefoperazone/sulbactam,piperacillin/sulbactam and other antibacterial drugs.Conclusion Detection and analysis of main pathogenic bacteria and sensitivity spectrum to commonly used drugs in our hospital,has important guiding significance for controlling hospital infections and diagnosis and treatment of infected patients.
