健康体检人群血同型半胱氨酸、血尿酸水平对高血压的预测作用

摘　　要：目的探讨健康体检人群检测血同型半胱氨酸、血尿酸水平对高血压的预测作用。方法选取我院2021年1月至2022年12月期间350例健康体检者资料进行回顾性分析,对350例健康体检者均进行血同型半胱氨酸、血尿酸水平检测,根据体检者是否发生高血压疾病进行分组。对比发生组与未发生组体检者血同型半胱氨酸、血尿酸水平,并对发生组高血压患者疾病进行分级,对比不同级别血同型半胱氨酸、血尿酸水平,以及采用ROC曲线分析其诊断效能。结果350例进行健康体检者中存在64例发生高血压疾病,发生组体检者血同型半胱氨酸、血尿酸水平均高于未发生组,对比具有统计学差异(P<0.05);血同型半胱氨酸ROC曲线下降面积(AUC)为0.697,血尿酸的AUC为0.591,在高血压疾病预测中具有一定的诊断价值,将之联合,AUC可达到0.836,联合检测诊断效果更为理想;血压分级为3级的患者血同型半胱氨酸、血尿酸水平均高于1级和2级,血压分级为2级的患者血同型半胱氨酸、血尿酸水平均高于1级(P<0.05)。结论血同型半胱氨酸、血尿酸水平高血压疾病预测、评估中具有一定的价值,血同型半胱氨酸、血尿酸水平在高血压患者中会明显升高,并且根据疾病严重程度呈正相关,对于预防及预测高血压疾病发生、进展具有十分重要的作用。Objective To investigate the predictive effect of blood homocysteine and uric acid levels on hypertension in healthy people.Methods The data of 350 healthy subjects in our hospital from January 2021 to December 2022 were retrospectively analyzed.Blood homocysteine and blood uric acid levels were detected in all 350 healthy subjects,and the subjects were grouped according to whether they had hypertension.Blood homocysteine and blood uric acid levels were compared between the occurrence group and the non-occurrence group,the disease of hypertension patients in the occurrence group was graded,and the blood homocysteine and blood uric acid levels of different levels were compared,and the diagnostic efficiency was analyzed by ROC curve.Results 64 of the 350 healthy subjects developed hypertension.The levels of blood homocysteine and uric acid in the occurrence group were higher than those in the non-occurrence group(P<0.05).The decreasing area of ROC curve(AUC)of blood homocysteine was 0.697,and the AUC of blood uric acid was 0.591,which had certain diagnostic value in the prediction of hypertension diseases.When combined,the AUC could reach 0.836,and the combined detection and diagnosis effect was more ideal.Blood homocysteine and uric acid levels in patients with blood pressure classification of grade 3 were higher than those of grade 1 and grade 2,blood levels of homocysteine and uric acid in patients with blood pressure grade 2 were higher than those of grade 1(P<0.05).Conclusions Blood homocysteine and blood uric acid levels are of certain value in the prediction and evaluation of hypertensive diseases.Blood homocysteine and blood uric acid levels are significantly increased in hypertensive patients,and are positively correlated with the severity of the disease,which plays a very important role in preventing and predicting the occurrence and progression of hypertensive diseases.
