摘　　要：甲状腺结节隶属于中医学“瘿病”“瘿瘤”“瘿气”范畴,多为肝失疏泄,气机郁滞,以致气、痰、瘀壅结于颈前。其核心病位在肝,多涉及脾、肾,痰、瘀、火、虚为其主要病理因素。“木郁达之”首见于《黄帝内经》,即针对木气郁结导致的异常气候变化对机体造成的一系列相关疾病,治以疏通畅达、调肝理气,补阴血而敛肝气,在升发梳理肝木之气、顺应木性的基础上使各脏腑功能恢复正常。因此,对甲状腺结节的治疗当从“治肝”入手,灵活运用疏肝、柔肝之法,并辅以清解郁热、化痰软坚、活血化瘀等方法,开其郁遏之气,使人体升降之气疏泄有序,气血调和,则结节自消。Thyroid nodule belongs to the categories of“ying disease”,“ying tumor”and“ying Qi” in traditional Chinese medicine.The thyroid nodule is caused by the failure of liver to convey and disperse and stagnation of Qi activity,resulting in stagnated Qi,phlegm and blood stasis in the front of the neck.The core disease location is liver,mainly involving spleen and kidney.Phlegm,stasis,fire and deficiency are the main pathological factors.“Facilitating the stagnant wood”first appears in Huangdi Neijing,that is,for the abnormal climate change caused by stagnant wood,a series of related diseases can be caused to the body.The treatment principle is to dredge and smooth the liver and regulate the liver-Qi.Yin blood is nourished to restrain liver-Qi.On the basis of smoothing and ascending liver-Qi and conforming to the wood nature,the functions of the viscera are restored to normal.Therefore,the treatment of thyroid nodule should start from the treatment of the liver,flexibly using the method of soothing the liver and softening the liver,and supplement by the methods of clearing stagnated heat,resolving phlegm and softening masses,promoting blood circulation and removing blood stasis,etc.,to unblock the stagnated Qi.These can make the Qi of the human body ascend and descend in an orderly manner and harmonize Qi and blood,bringing on the elimination of thyroid nodule.
