摘 要:目的探讨超声在婴幼儿髋关节发育异常(DDH)中的筛查价值以及与髋关节超声图像特点及多个超声测量参数的相关性。方法回顾性收集2021年1月至2022年3月在西安市人民医院就诊的8149例进行双侧髋关节检查的婴幼儿临床资料,所有患者均行超声检查。统计DDH发生状况,并依据DDH发生结果将婴幼儿分为正常组与异常组,比较两组间髋关节α角、β角、股骨头覆盖率(FHC)。分析异常组中不同性别、发病部位、皮纹状况及月龄间髋关节α角、β角、FHC水平差异,并探讨髋关节α角、β角、FHC的相关性。结果8149例婴幼儿16298个髋关节中,出现异常有79个(4.84‰),设为异常组,余下16219个设为正常组。正常组α角、FHC均高于异常组,β角低于异常组,差异具有统计学意义(P<0.01)。经统计分析,女性婴幼儿DDH发生率高于男性婴幼儿,左侧髋关节DDH发生率高于右侧髋关节,皮纹不对称婴幼儿DDH发生率高于皮纹对称婴幼儿,0~2月龄婴幼儿DDH发生率高于>2~4月龄、>4~6月龄,差异均具有统计学意义(P<0.01)。经Spearman相关性分析,α角与FHC呈正相关,与β角呈负相关;β角与α角、FHC均呈负相关;FHC与α角呈正相关,与β角负相关(P<0.05)。结论较髂关节发育正常的婴幼儿,DDH婴幼儿超声图像特点差异显著,多种超声测量参数间具有相关性,超声可为临床筛查DDH提供科学参考。Objective To investigate the value of ultrasound in screening infantile developmental dysplasia of the hip(DDH),the ultrasonic image characteristics of hip joint,and correlation of various ultrasonic measurement parameters.Methods The clinical data of 8149 infants who received bilateral hip joint examination in Xi an People s Hospital from January 2021 to March 2022 were retrospectively collected.All infants underwent ultrasound examination.The incidence of DDH was analyzed,and the infants were divided into normal group and abnormal group according to the presence or absence of DDH.The hipαangle,βangle and femoral head coverage(FHC)were compared between the two groups.The hipαangle,βangle,and FHC were analyzed in different gender,pathogenic sites,skin lines,and age in the abnormal group,and the correlation of hipαangle,βangle,and FHC was discussed.Results Among 16298 hip joints of 8149 infants,79(4.84‰)were abnormal,which were defined as the abnormal group,and the remaining 16219 were defined as the normal group.αangle and FHC of the normal group were larger than those of the abnormal group,andβangle was smaller than that of the abnormal group,with statistically significant difference(P<0.01).According to statistical analysis,the incidence of DDH was higher in female infants than in male ones,and higher in the left hip joint than in the right one,higher in infants with asymmetric skin lines than in infants with symmetric skin lines,and higher in 02 month-old infants than in>24 month-old and>46 month-old ones,with statistically significant difference(P<0.01).Spearman correlation analysis found thatαangle was positively correlated with FHC,and negatively correlated withβangle.βangle was negatively correlated withαangle and FHC.FHC was positively correlated withαangle and negatively correlated withβangle(P<0.05).Conclusion Compared with infants with normal hip joint development,the ultrasonic image characteristics of infants with DDH are significantly different,and there are correlations among variou
