摘 要:目的分析超声引导神经阻滞复合全身麻醉在膝关节置换术中应用的效果。方法便利选取2020年1月—2023年5月江苏大学附属武进医院(徐州医科大学武进临床学院)收治的83例膝关节置换术治疗患者为研究对象,随机数表法的方式分为对照组(41例)和观察组(42例)。对照组采用传统全身麻醉,观察组采用超声引导神经阻滞复合全身麻醉,比较两组患者手术情况,麻醉前、插管时、拔管时的术中血流动力学变化,术后6、24、48 h镇痛效果。结果观察组患者苏醒时间(6.34±1.28)min和拔管时间(11.25±3.26)min,均短于对照组,差异有统计学意义(t=4.550、9.101,P<0.05)。观察组插管时和拔管时心率和平均动脉压水平均低于对照组,差异有统计学意义(P<0.05);观察组术后视觉模拟评分低于对照组,差异有统计学意义(P<0.05)。结论膝关节置换术中采用超声引导神经阻滞复合全身麻醉的方式的应用价值较佳。Objective To analyze the effect of ultrasound-guided nerve block composite general anesthesia in the application of knee arthroplasty.Methods 83 patients were admitted to Wujin Hospital Affiliated to Jiangsu University(Wujin Clinical College of Xuzhou Medical University)for treatment of knee arthroplasty from January 2020 to May 2023 were conveniently selected as study objects,and were divided into the control group(41 cases)and the observation group(42 cases)by the grouping of randomized numerical tables.The control group used traditional general anesthesia,and the observation group used ultrasound-guided nerve block composite general anesthesia.The operation conditions,intraoperative hemodynamic changes before anesthesia,at the time of intubation and extubation,and the analgesic effects at 6 h,24 h and 48 h after operation were compared between the two groups.Results The awakening time(6.34±1.28)min and extubation time(11.25±3.26)min of patients in the observation group were shorter than those in the control group,and the difference was statistically significant(t=4.550,9.101,P<0.05).Heart rate and mean arterial pressure levels at intubation and extubation in the observation group were lower than those in the control group,and the difference was statistically significant(P<0.05).The postoperative Visual Analogue Scale of the observation group were lower than that of the control group,and the difference was statistically significant(P<0.05).Conclusion The application of ultrasound-guided nerve block composite general anesthesia in knee arthroplasty has better value.
