瑞芬太尼复合丙泊酚静脉麻醉在宫腔镜手术中的应用价值分析

摘　　要：目的 探讨瑞芬太尼复合丙泊酚静脉麻醉在宫腔镜手术中的应用价值。方法 随机选取2021年1月—2022年12月江苏省淮安市中医院需行宫腔镜手术的患者80例,采用随机数表法分为瑞芬太尼组和芬太尼组,每组40例,均予以丙泊酚输注,全程记录患者生命体征变化,观察术中丙泊酚用量、麻醉效果、苏醒时间及应激反应。结果 瑞芬太尼组麻醉优良率为97.50%,较芬太尼组的80.00%高,差异有统计学意义(χ^(2)=4.507,P<0.05)。瑞芬太尼组麻醉药物用量较芬太尼组少,苏醒时间较芬太尼组短,差异有统计学意义(P<0.05)。手术开始时、术后20 min时瑞芬太尼组平均动脉压、心率水平较芬太尼组低,差异有统计学意义(P<0.05)。结论 瑞芬太尼复合丙泊酚静脉麻醉应用于宫腔镜手术中,可提高麻醉效果,减少术中丙泊酚用量,缩短苏醒时间,减轻应激反应,值得临床应用推广。Objective To explore the application value of remifentanil compound propofol intravenous anesthesia in hysteroscopic surgery.Methods A total of 80 patients requiring hysteroscopic surgery in Huai'an Hospital of Traditional Chinese Medicine,Jiangsu Province from January 2021 to December 2022 were randomly selected.They were divided into remifentanil group and fentanyl group by random number table method,with 40 cases in each group.All patients were given propofol infusion and the changes of vital signs were recorded.The amount of propofol,anesthetic effect,recovery time and stress reaction were observed.Results The rate of excellent and good anesthesia in remifentanil group was 97.50%,higher than that in fentanyl group(80.00%),and the difference was statistically significant(χ^(2)=4.507,P<0.05).The dosage of narcotic drugs in remifentanil group was lower than that in fentanyl group,and the recovery time was shorter than that in fentanyl group,and the difference was statistically significant(P<0.05).At T2 and T3,the mean arterial pressure and heart rate in remifentanil group were lower than those in fentanyl group,and the difference was statistically significant(P<0.05).Conclusion The application of remifentanil compound propofol intravenous anesthesia in hysteroscopic surgery can improve the anesthetic effect,reduce the amount of intraoperative propofol,shorten the awakening time,reduce the stress reaction.
