中药内服联合针刺及正骨推拿治疗腰椎间盘突出症临床观察

摘　　要：目的探讨中药内服联合针刺及正骨推拿治疗腰椎间盘突出症(LDH)的效果。方法选取2019年11月—2021年11月于宜春市第二人民医院就诊的LDH患者70例,按随机数字表法分为对照组和观察组,各35例。所有患者均予以常规西医治疗,对照组于此基础上辅以正骨推拿,观察组于对照组基础上加用中药活血通络汤内服及针刺治疗,连续治疗4周。对比2组临床疗效、下肢感觉障碍改善情况、自由基代谢与不良反应。结果观察组治疗总有效率为94.29%(33/35),高于对照组的77.14%(27/35);下肢感觉障碍改善总有效率为91.43%(32/35),高于对照组的71.43%(25/35),差异均有统计学意义(P<0.05)。治疗前2组超氧化物歧化酶(SOD)、丙二醛(MDA)比较,差异无统计学意义(P>0.05);治疗后,观察组MDA低于对照组,SOD高于对照组,差异均有统计学意义(P<0.05)。2组患者治疗期间均无不良反应发生。结论中药内服联合针刺及正骨推拿治疗LDH患者效果确切,可有效改善其下肢感觉障碍情况、调节自由基代谢,且安全性高。Objective To investigate the therapeutic effect of oral administration of Chinese medicine combined with acupuncture and bone-setting massage in patients with lumbar disc herniation(LDH).Methods A total of 70 patients with LDH who were treated in The Second People’s Hospital of Yichun from November 2019 to November 2021 were selected and divided into two groups according to the random number table method,with 35 cases in each group.All patients were treated with conventional Western medicine,the control group was supplemented with bone-setting massage,and the observation group was additionally treated with Chinese medicine Huoxue Tongluo decoction and acupuncture on the basis of the control group for 4 weeks.The clinical efficacy,improvement of lower limb sensory disturbance,free radical metabolism and adverse reactions were compared between the two groups.Results The total effective rate of the observation group was 94.29%(33/35),which was higher than that of the control group(77.14%,27/35).The total effective rate was 91.43%(32/35),which was higher than 71.43%(25/35)in the control group,and the differences were statistically significant(P<0.05).There was no significant difference in superoxide dismutase(SOD)and malondialdehyde(MDA)between the two groups before treatment(P>0.05).After treatment,MDA and SOD in observation group were lower than control group,and the differences were statistically significant(P<0.05).No adverse reactions occurred in 2 groups during treatment.Conclusion Oral administration of Chinese medicine combined with acupuncture and bone-setting massage is effective in the treatment of LDH patients.It can effectively improve the sensory disturbance of the lower extremities,regulate the metabolism of free radicals,and accelerate the recovery of patients with high safety.
