摘　　要：目的探讨丙酸氟替卡松联合布地奈德对慢性咳嗽患儿肺功能、炎症指标的影响。方法选取2020年3月至2021年3月新沂市人民医院诊治的60例慢性咳嗽患儿作为研究对象,随机分为对照组与研究组,每组30例。对照组给予丙酸氟替卡松治疗,研究组给予丙酸氟替卡松联合布地奈德治疗。比较两组呼气峰流速(PEF)、用力肺活量(FVC)、第1秒用力呼气容积(FEV1)、FEV1/FVC、肿瘤坏死因子α(TNF-α)、白细胞介素-4(IL-4)、IgE、不良反应发生率及临床疗效。结果治疗后,研究组PEF快于对照组,FVC、FEV1均大于对照组,FEV1/FVC高于对照组,差异有统计学意义(P<0.05)。治疗后,研究组TNF-α、IL-4、IgE水平均低于对照组,差异有统计学意义(P<0.05)。研究组不良反应发生率略高于对照组,但差异无统计学意义。研究组治疗总有效率高于对照组,差异有统计学意义(P<0.05)。结论丙酸氟替卡松联合布地奈德治疗慢性咳嗽效果显著,可改善肺功能及TNF-α、IgE、IL-4水平,值得临床推广应用。Objective To explore the effects of fluticasone propionate combined with budesonide on lung function and inflammatory index in children with chronic cough.Methods 60 children with chronic cough admitted to Xinyi City People's Hospital from March 2020 to March 2021 were selected as the research subjects,and they were randomly divided into the control group and the research group,with 30 cases in each group.The control group was treated with fluticasone propionate,while the research group was treated with fluticasone propionate combined with budesonide.The peak expiratory flow(PEF),forced vital capacity(FVC),forced expiratory volume(FEV1),FEV1/FVC,tumor necrosis factor-α(TNF-α),interleukin-4(IL-4),IgE,the incidence of adverse reactions and clinical efficacy were compared between the two groups.Results After treatment,PEF in the research group was faster than that in the control group,FVC and FEV1 were higher than those in the control group,and FEV1/FVC was higher than that in the control group,the differences were statistically significant(P<0.05).After treatment,the levels of TNF-α,IL-4 and IgE in the research group were lower than those in the control group,and the differences were statistically significant(P<0.05).The incidence of ad-verse reactions in the research group was lower than that in the control group,but the difference was not statistically significant(P<0.05).The total ef-fective rate in the research group was higher than that in the control group,and the difference was statistically significant(P<0.05).Conclusion Fluti-casone propionate combined with budesonide is effective in the treatment of chronic cough,which can effectively reduce the occurrence of adverse re-actions in children,improve lung function and TNF-α,IgE and IL-4 levels,which is worthy of clinical promotion and application.
