摘　　要：目的探讨椎弓根钉棒固定联合椎间融合器植骨融合(IFCBGF)治疗腰椎管狭窄症(LSS)对腰椎-骨盆平衡的改善作用。方法选取2019年1月至2021年5月萍乡市第三人民医院收治的60例LSS患者作为研究对象,按照手术方式的不同分为常规组(单纯椎板切除减压,n=29)和联合组(采用椎弓根钉棒固定联合IFCBGF治疗,n=31)。比较两组手术疗效及治疗前后腰椎-骨盆平衡指标改善情况。结果联合组手术总有效率为90.32%,高于常规组的68.97%,差异有统计学意义(P<0.05)。治疗前,两组腰椎前凸角(LL)、椎间高度(IH)、骶骨倾斜角(SS)、骨盆入射角(PI)、骨盆倾斜角(PT)比较差异无统计学意义;治疗后,两组LL、SS均大于治疗前,IH均高于治疗前,PT均小于治疗前,且联合组LL、SS均大于常规组,IH高于常规组,PT小于常规组,差异有统计学意义(P<0.05),两组PI比较差异无统计学意义。结论椎弓根钉棒固定联合IFCBGF治疗LSS能有效提升手术疗效,恢复腰椎正常生理前凸和高度,促进腰椎-骨盆平衡改善,值得临床推广应用。Objective To investigate the effect of pedicle screw rod fixation combined with interbody fusion cage bone graft fusion(IFCBGF)in the treatment of lumbar spinal stenosis(LSS)on the improvement of lumbar pelvic balance.Methods 60 patients with LSS admitted to the Third People's Hospital of Pingxiang from January 2019 to May 2021 were selected as the research subjects,they were divided into the conventional group(simple laminectomy and decompression,n=29)and combined group(pedicle screw rod fixation combined with IFCBGF,n=31).The total ef-fective rate and the improvement of lumbar pelvic balance indexes before and after treatment were compared between the two groups.Results The total effective rate of surgery in the combined group was 90.32%,which was higher than 68.97%in the conventional group,and the difference was statistically significant(P<0.05).Before treatment,there were no significant differences in lumbar lordosis angle(LL),intervertebral height(IH),sacral slope angle(SS),pelvic incidence angle(PI)and pelvic tilt angle(PT)between the two groups;after treatment,LL and SS of the two groups were higher than those before treatment,IH were higher than that before treatment,PT were lower than that before treatment,and LL and SS in the combined group were higher than those in the conventional group,IH was higher than that in the conventional group,PT was lower than that in the conventional group,the differences were statistically significant(P<0.05),there was no significant difference in PI between the two groups.Conclusion Pedicle screw fixation combined with IFCBGF in the treatment of LSS can effectively improve the surgical efficacy,restore the nor-mal physiological lordosis and height of the lumbar spine,and promote the improvement of lumbar pelvic balance,it is worthy of clinical promo-tion and application.
