摘 要:目的观察跨理论模型康复理念联合穴位贴敷对髋关节置换术患者术后恢复、疼痛及股静脉血流速度的影响。方法将98例髋关节置换术患者按照随机数字表法分为2组,对照组49例予常规骨科干预,治疗组49例予跨理论模型康复理念联合穴位贴敷干预。观察2组术前、干预6个月后Harris髋关节功能评分量表(Harris)评分变化;比较2组术后恢复情况;比较2组术前及术后第3、7天疼痛视觉模拟评分(VAS)变化;比较2组术前、术后第7天股静脉血流峰速、血流平均速度变化。结果2组干预6个月后Harris评分均较本组术前升高(P<0.05),治疗组干预6个月后Harris评分高于对照组(P<0.05)。治疗组下床活动时间、拆线时间、住院时间均短于对照组(P均<0.05)。2组术后第3、7天疼痛VAS均较本组术前降低(P<0.05),2组术后第7天疼痛VAS均较本组术后第3天降低(P<0.05),治疗组术后第3、7天疼痛VAS均低于对照组同期(P<0.05)。2组术后第7天股静脉血流峰速、血流平均速度均较本组术前升高(P<0.05),治疗组术后第7天股静脉血流峰速、血流平均速度均高于对照组(P<0.05)。结论对髋关节置换术患者术后进行跨理论模型康复理念联合穴位贴敷干预,能促进患者术后恢复,缓解疼痛,提高髋关节功能,促进股静脉血液流动。Objective To observe the effects of the trans-theoretical model rehabilitation concept combined with acupoint patching on postoperative recovery,pain and blood flow velocity of the femoral veins in patients underwent hip replacement.Methods Ninety-eight patients underwent hip replacement were randomly assigned to the control group(n=49)and the treatment group(n=49),in which they received conventional orthopedic intervention and trans-theoretical model rehabilitation concept combined with acupoint patching,respectively.The key observation was Harris Hip Score(HHS)preoperatively and 6 months after intervention;postoperative recovery;visual analogue scale(VAS)for pain preoperatively and at 3 and 7 days postoperatively;peak velocity and mean blood velocity of the femoral veins preoperatively and at 7 days postoperatively.Results After 6 months of intervention,HHS in the both groups was significantly higher than those before surgery(P<0.05),which in the treatment group was significantly higher than the control group(P<0.05).The ambulation time,take out stitches time,hospitalization time in the treatment group were significantly shorter than those in the control group(all P<0.05).VAS scores at 3 and 7 days postoperatively in the both groups were significantly decreased(P<0.05),which in the treatment group were significantly lower than the control group(P<0.05).VAS scores at 7 days postoperatively in the both groups were significantly lower than that at 3 days postoperatively(P<0.05).The peak velocity and mean blood velocity of the femoral veins at 7 days postoperatively in the both groups were significantly increased(P<0.05),the increase in the treatment group was more common than the control group(P<0.05).Conclusion For patients underwent hip replacement,trans-theoretical model rehabilitation concept combined with acupoint patching can promote postoperative recovery,relieve pain,improve hip joint function,and promote blood flow velocity of the femoral veins.
