基于数据挖掘探析刺络拔罐法的临床应用规律

摘　　要：目的基于古今医案云平台(V 2.3.5)挖掘刺络拔罐法的临床应用规律及特点,为临床提供参考。方法计算机搜索中国知网、维普网、万方数据知识服务平台收录的2013年1月1日—2023年1月1日刺络拔罐法临床相关文献。用Excel表格建立数据库并录入古今医案云平台(V 2.3.5),总结刺络拔罐法治疗的疾病及其分类、腧穴选取、操作过程、联合疗法的应用特点及规律。结果刺络拔罐法多用于治疗皮肤科、外科疾病;治疗疾病频次前7位依次为痤疮、带状疱疹、腰椎间盘突出症、肩关节周围炎、带状疱疹后遗神经痛、面神经炎、急性腰扭伤。腧穴应用频次前10位分别为阿是穴、大椎、肺俞、委中、膈俞、心俞、脾俞、肝俞、肾俞、血海。刺络针具多应用三棱针;刺络方式多选用点刺;拔罐罐具多选取玻璃罐;留罐时间多为5~10 min;大部分文献未提及出血量是多少,44篇文献中出血量为3~5 mL。多配合针灸治疗相关疾病。结论刺络拔罐法多用于治疗皮肤科、外科疾病,有其独特疗法与优势,可用来指导临床实践。Objective To explore the clinical application rules and characteristics of blood-letting puncture and cupping therapy based on the ancient and modern medical record cloud platform(V 2.3.5),thus providing reference for clinical practice.Methods Relevant clinical literatures of blood-letting puncture and cupping therapy published from January 2013 to January 2023 were searched in the China Knowledge Resources Database(CNKI),the Chinese Science and Technology Periodical Database(VIP)and Wanfang Data Knowledge Service Platform by computer.A database was created using Excel and data were recorded into the cloud platform of ancient and modern medical records(V 2.3.5).Application rules of diseases treated by blood-letting puncturing and cupping and its classification,acupoint selection,operation process and combined therapy.Results The blood-letting puncture and cupping therapy was often used to the treatment of surgical and dermatological diseases.The top 7 dominant diseases were acne,herpes zoster,lumbar disc herniation,shoulder periarthritis,postherpetic neuralgia,facial nerve paralysis,and acute lumbar sprain.The top 10 acupoints were Ashi acupoint,Dazhui(DU14),Feishu(BL13),Weizhong(BL40),Geshu(BL17),Xinshu(BL15),Pishu(BL20),Ganshu(BL18),Shenshu(BL23)and Xuehai(SP10).Three-edged needle was frequently used as collateral puncture needle.Pricking was frequently used as puncture method.Glass cans were frequently used as cupping utensil.The tank retention time was usually 5~10 min.Most of the literature did not mention the amount of bleeding,which,in 44 articles ranged 3~5 mL.In combination therapy,acupuncture and moxibustion were frequently combined to treat related diseases.Conclusion The blood-letting puncture and cupping therapy is mostly used to the treatment of surgical and dermatological diseases.It has its unique treatment and advantages and can be used to guide clinical practice.
