水凝胶冷敷贴对减少功能性鼻内镜手术 患者术中出血的效果研究

摘　　要：目的:探讨水凝胶冷敷贴冷敷对减少功能性鼻内镜患者术中出血的效果。方法:选取80例慢性鼻-鼻窦炎行功能性鼻内镜患者为研究对象,按随机数字表法随机分为实验组和对照组,每组40例。对照组术中采用浸有1∶10000肾上腺素棉片填塞鼻腔,实验组在对照组的基础上采用适合患者的蝶翼型水凝胶冷敷贴(4℃冰箱内放置12 h)贴于额鼻部直至手术结束。结果:实验组和对照组术中出血量分别为(109.73±50.50)ml和(152.98±55.24)ml(P<0.001),术中心率分别为(73.28±7.82)次·min^(-1)和(81.80±6.96)次·min^(-1)(P<0.001),手术时间分别为(81.80±6.96)min及(104.75±25.8)min(P<0.001)。术后疼痛程度实验组也明显优于对照组(P<0.0153)。结论:水凝胶冷敷贴冷敷能有效减少功能性鼻内镜患者术中出血,降低肾上腺素对患者心率的影响,缩短手术时间,减轻患者术后疼痛程度。Objective:To investigate the effect of hydrogel cold compress on reducing intraoperative bleeding in patients with functional nasal endoscopy.Methods:80 patients with chronic rhinosinusitis receiving functional nasal endoscopy were randomly divided into experimental group and control group according to random number table method,with 40 cases in each group.In the control group,the nasal cavity was filled with 1∶10000 epinephrine cotton tablets.In the experimental group,on the basis of the control group,the butterfly wing type hydrogel cold compress paste which was suitable for patients was applied to the forehead and nose until the end of the operation.Results:The intraoperative blood loss in the experimental group and the control group was(109.73±50.50)ml and(152.98±55.24)ml(P<0.001),and the intraoperative heart rate was(73.28±7.82)times per minute and(81.80±6.96)times per minute(P<0.001),respectively.The operation time was(81.80±6.96)minutes and(104.75±25.8)minutes respectively(P<0.001).The degree of postoperative pain relief in the experimental group was also significantly better than that in the control group(P<0.0153).Conclusion:Hydrogel cold compress can effectively reduce intraoperative bleeding in patients with functional nasal endoscopy and the effect of adrenalin on patients heart rate,shorten the operation time and reduce the degree of postoperative pain.
