摘　　要：目的 探讨阻塞性睡眠呼吸暂停综合征患者心律失常发生情况及血气指标的相关性。方法 选取选取2022年1月~2022年12月于我院就诊的阻塞性睡眠呼吸暂停综合征患者195例。依据患者的AHI分为轻度组(n=48,AHI:5~15)、中度组(n=63,AHI:16~30)、重度组(n=84,AHI> 30)。比较各组之间心律失常发生情况及HRV[全部正常RR间期的标准差(SDNN)、低频功率(LF)、高频功率(HF)]与AHI、血气指标[夜间最低血氧饱和度(LaSO_(2))、夜间平均血氧饱和度(MSaO_(2))、氧饱和度低于90%的时间占监测时间的百分比(Ts90)、氧减指数(ODI)]的相关性。结果 重度组心律失常发生率(73.81%)、中度组(69.64%)(P> 0.05),但均高于轻度组(41.67%)(P <0.05);相关性分析显示HRV各参数与AHI以及血气指标各参数均相关(P <0.05)。结论 阻塞性睡眠呼吸暂停综合征患者病情越严重发生心律失常可能性越高,交感、迷走神经失衡越明显,夜间血氧下降幅度越大。Objective Toinvestigate the correlation of arrhythmia occurrence and blood gas index in patients with obstructive sleep apnea syndrome.MethodsThe clinical data of 195 patients with OSAS in our hospital were collected for study from January 2022 to December 2022.According to AHI,patients were divided into mild group(n=48,AHI:5-15),moderate group(n=63,AHI:16-30)and severe group(n=84,AHI>30).Arrhythmia occurrence and HRV[standard deviation of NN intervals(SDNN),low frequency power(LF),high frequency power(HF)]and AHI,blood gas indexes[minimum nighttime blood oxygen saturation(LaS),mean nighttime blood oxygen saturation(MSaO_(2)),oxygen saturation<90%as a percentage of the monitoring time(Ts90),oxygen depletion index(ODI)]in different groups were compared.Resuits The incidence of arrhythmia in severe group(73.81%)and moderate group(69.64%)(P>0.05)was higher than that in mild group(41.67%)(P<0.05).Correlation analysis showed that HRV parameters were correlated with AHI and blood gas indexes(P<0.05).Conclusion The more severe the OSAS in patients,the higher the incidence of arrhythmia,the more obvious the imbalance of sympathetic and vagus nerves and the greater the decrease of nighttime blood oxygen.
